NEUROSURGERY. Frequency and Factors Associated With Unsuccessful Lead (First) Coil Placement in Patients Undergoing Coil Embolization of Intracranial Aneurysms

Khatri, Rakesh MD; Chaudhry, Saqib A. MD; Rodriguez, Gustavo J. MD; Suri, M. Fareed K. MD; Cordina, Steve M. MD; Qureshi, Adnan I. MD

BACKGROUND: There is limited knowledge about associated rates, aneurysm characteristics, technical factors, and immediate impact of unsuccessful placement of the lead (first) coil during endovascular embolization of intracranial aneurysms. OBJECTIVE: To determine the rates, associated risk factors, and consequences of lead coil placement failure in consecutive embolization procedures.

JOURNAL OF NEUROINTERVENTIONAL SURGERY. Utilization of the Navien distal intracranial catheter in 78 cases of anterior circulation aneurysm treatment with the Pipeline embolization device

Geoffrey P Colby, Li-Mei Lin, Judy Huang, Rafael J Tamargo, Alexander L Coon

Background Endovascular treatment of intracranial aneurysms relies on coaxial catheter support systems to provide safe and stable access. Large-bore distal intracranial catheters have become necessary for aneurysm treatment with flow diverting devices including the Pipeline embolization device (PED). These catheters must accommodate 0.027 inch microcatheters, be supple enough to track distally and be able to provide sufficient support for manipulations required for PED deployment.

CARDIOVASCULAR AND INTERVENTIONAL RADIOLOGY. Elastoplasty: First Experience in 12 Patients

Thijs A. J. Urlings, Edwin van der Linden

Purpose: Percutaneous vertebroplasty with polymethylmethacrylate (PMMA) is used increasingly for pain relief in symptomatic neoplastic or osteoporotic compression fractures. However, restoration of the stiffness of the treated vertebrae might propagate secondary fracture of adjacent vertebrae. Elastoplasty might prevent these secondary fractures. We assessed retrospectively our experience with elastoplasty in 12 patients, focusing on silicone migration.

CIRCULATION. Genetic Determinants of Dabigatran Plasma Levels and Their Relation to Bleeding

Guillaume Paré, MD; Niclas Eriksson, PhD; Thorsten Lehr, PhD; Stuart Connolly, MD; John Eikelboom, MD; Michael D. Ezekowitz, MD, PhD; Tomas Axelsson, PhD; Sebastian Haertter, PhD; Jonas Oldgren, MD, PhD; Paul Reilly, PhD; Agneta Siegbahn, MD, PhD; Ann-Christine Syvanen, PhD; Claes Wadelius, MD, PhD; Mia Wadelius, MD, PhD; Heike Zimdahl-Gelling, PhD; Salim Yusuf, MD; Lars Wallentin, MD, PhD

Background—Fixed-dose unmonitored treatment with dabigatran etexilate is effective and has a favorable safety profile in the prevention of stroke in atrial fibrillation patients compared with warfarin. We hypothesized that genetic variants could contribute to interindividual variability in blood concentrations of the active metabolite of dabigatran etexilate and influence the safety and efficacy of dabigatran.

NEURORADIOLOGY. Treatment of intracranial aneurysms. Reconstruction of the parent artery with flow-diverting (Silk) stent

Aase Wagner, Marie Cortsen, John Hauerberg, Bertil Romner, Mathias Pedersen Wagner

Introduction: Since the flow diverters (FDs) have been introduced it is possible to treat aneurysms that are considered difficult or impossible to treat with usual endovascular or surgical methods. It is still uncertain which aneurysms are suitable for this new treatment. We present the periprocedural complications, immediate result, late complications, imaging follow-up at 6 and 12 months and clinical follow-up at 2–23 months.

JOURNAL OF NEURORADIOLOGY. Carotid artery stenting in patients with symptomatic carotid stenosis: A single-center series

I. Kessler, B. Gory, F. Macian, G. Nakiri, M. Al-Khawaldeh, R. Riva, M.P. Boncoeur, C. Mounayer

Objectives: Carotid angioplasty with stenting (CAS) in patients with carotid stenosis (CS) has become more restricted in France especially since the disclosure of such studies as EVA-3S and Stent-supported percutaneous angioplasty of the carotid artery versus endarterectomy (SPACE). This report is of a series of CS cases contraindicated for endarterectomy that underwent CAS at a French center of interventional neuroradiology.

JOURNAL OF VASCULAR SURGERY. Effect of hospital-level variation in the use of carotid artery stenting versus carotid endarterectomy on perioperative stroke and death in asymptomatic patients

Theodore H. Yuo, MD , Howard S. Degenholtz, PhD , Rabih A. Chaer, MD , Kevin L. Kraemer, MD, MSc , Michel S. Makaroun, MD

Objective: Perioperative stroke and death (PSD) are more common after carotid artery stenting (CAS) than after carotid endarterectomy (CEA) in symptomatic patients, but whether this is also true in asymptomatic patients is unclear. Furthermore, use of both CEA and CAS varies geographically, suggesting possible variation in outcomes. We compared odds of PSD after CAS and CEA in asymptomatic patients to determine the impact of this variation.

JOURNAL OF VASCULAR SURGERY. The risk of carotid artery stenting compared with carotid endarterectomy is greatest in patients treated within 7 days of symptoms

Barbara Rantner, MD, PhD , Georg Goebel, PhD , Leo H. Bonati, MD , Peter A. Ringleb, MD , Jean-Louis Mas, MD , Gustav Fraedrich, MD , Carotid Stenting Trialists Collaboration

Objective: Among patients with symptomatic carotid artery stenosis, carotid artery stenting (CAS) is associated with a higher risk of periprocedural stroke or death than carotid endarterectomy (CEA). Uncertainty remains whether the balance of risk changes with time since the most recent ischemic event.
