NEURORADIOLOGY. Retrograde stenting through the posterior cerebral artery in coil embolization of the posterior communicating artery aneurysm

Young Dae Cho, Kang Min Kim, Woong Jae Lee, Hyun-Seung Kang, Jeong Eun Kim, Moon Hee Han

Introduction: Protection techniques using stents or balloons are occasionally limited in coil embolization of wide-necked posterior communicating artery (PcomA) aneurysms in which the PcomA originated from the aneurysm neck at an acute angle. Here, we present two cases undergoing retrograde stenting through the posterior cerebral artery in coil embolization of the PcomA aneurysms.

THE LANCET NEUROLOGY. Targeted use of heparin, heparinoids, or low-molecular-weight heparin to improve outcome after acute ischaemic stroke: an individual patient data meta-analysis of randomised controlled trials

Dr William N Whiteley PhD , Harold P Adams MD , Philip MW Bath FRCP, Eivind Berge MD , Per Morten Sandset MD , Martin Dennis MD , Gordon D Murray PhD , Ka-Sing Lawrence Wong MD , Peter AG Sandercock MD

Background: any international guidelines on the prevention of venous thromboembolism recommend targeting heparin treatment at patients with stroke who have a high risk of venous thrombotic events or a low risk of haemorrhagic events. We sought to identify reliable methods to target anticoagulant treatment and so improve the chance of avoiding death or dependence after stroke.

STROKE. Impact of Acute Cocaine Use on Aneurysmal Subarachnoid Hemorrhage

Tiffany R. Chang, MD; Robert G. Kowalski, MBBCh, MS; Filissa Caserta, MSN, ACNP-BC, CNRN; Juan Ricardo Carhuapoma, MD; Rafael J. Tamargo, MD; Neeraj S. Naval, MD

Background and Purpose—Acute cocaine use has been temporally associated with aneurysmal subarachnoid hemorrhage (aSAH). This study analyzes the impact of cocaine use on patient presentation, complications, and outcomes.

NEURORADIOLOGY. Flow diverter effect on cerebral aneurysm hemodynamics: an in vitro comparison of telescoping stents and the Pipeline

Breigh N. Roszelle, L. Fernando Gonzalez, M. Haithem Babiker, Justin Ryan, Felipe C. Albuquerque, David H. Frakes

Introduction: Flow diverting devices and stents can be used to treat cerebral aneurysms too difficult to treat with coiling or craniotomy and clipping. However, the hemodynamic effects of these devices have not been studied in depth. The objective of this study was to quantify and understand the fluid dynamic changes that occur within bifurcating aneurysms when treated with different devices and configurations.

AMERICAN JOURNAL OF NEURORADIOLOGY. Combined Treatment of Brain AVMs with Use of Onyx Embolization followed by Radiosurgery

L. Pierot, K. Kadziolka, F. Litré and P. Rousseaux

BACKGROUND AND PURPOSE: The treatment of cerebral AVMs is complex, reliant on interventions such as embolization, surgery, and radiosurgery, or a combination of these modalities. To date, treatment with the embolic agent Onyx, followed by radiosurgery, has not been evaluated. The goal of this study was to evaluate the safety and efficacy of this combination in a homogeneous, monocentric series.

AMERICAN JOURNAL OF NEURORADIOLOGY. Rate and Prognosis of Patients under Conscious Sedation Requiring Emergent Intubation during Neuroendovascular Procedures

A.E. Hassan, U. Akbar, S.A. Chaudhry, W.G. Tekle, R.P. Tummal, G.J. Rodriguez and A.I. Qureshi

BACKGROUND AND PURPOSE: Neuroendovascular procedures are performed with the patient under conscious sedation (local anesthesia) in varying numbers of patients in different institutions, though the risk of unplanned conversion to general anesthesia is poorly characterized. Our aim was to ascertain the rate of failure of conscious sedation in patients undergoing neuroendovascular procedures and compare the in-hospital outcomes of patients who were converted from conscious sedation to general anesthesia with those whose procedures were initiated with general anesthesia.

AMERICAN JOURNAL OF NEURORADIOLOGY. In Not Only Vertebroplasty but Also Kyphoplasty, the Resolution of Vertebral Deformities Depends on Vertebral Mobility

K. Yokoyama, M. Kawanishi, M. Yamada, H. Tanaka, Y. Ito, M. Hirano and T. Kuroiwa

BACKGROUND AND PURPOSE: It has not been clarified whether the postoperative resolution of vertebral deformities achieved by KP are superior to those achieved by VP. We compared the outcomes of KP and VP, taking into account the presence of vertebral mobility, to determine whether the balloon inflation in KP may contribute to postoperative resolution of the vertebral deformities in patients with VCFs.

Background and Purpose—Blood pressure (BP) lowering is often conducted as part of general acute management in patients with acute intracerebral hemorrhage. However, the relationship between BP after antihypertensive therapy and clinical outcomes is not fully known.

AMERICAN JOURNAL OF NEURORADIOLOGY. Safety and Efficacy of Neuroform for Treatment of Intracranial Aneurysms: A Prospective, Consecutive, French Multicentric Study

J.C. Gentrica, A. Biondib, M. Piotinc, C. Mounayerd, K. Lobotesise, A. Bonaféf and V. Costalatf for the French SENAT Investigators

BACKGROUND AND PURPOSE: Endovascular embolization of wide-neck intracranial aneurysms can be technically challenging, especially when the anatomy is complex. Stent reconstruction of the parent artery is commonly used to treat wide-neck and bifurcated aneurysms. The main objective of this study was to investigate the periprocedural and midterm morbidity and mortality results of this procedure.

STROKE. Endovascular Treatment of Intracranial Aneurysms in Elderly Patients. A Systematic Review and Meta-Analysi

Carmelo L. Sturiale, MD; Waleed Brinjikji, MD; Mohammad H. Murad, MD, MPH; Giuseppe Lanzino, MD

Background and Purpose—Use of endovascular coiling for treatment of ruptured and unruptured intracranial aneurysms (IAs) in the elderly is increasing. We performed a meta-analysis of the literature examining clinical and angiographic outcomes for treatment of IAs in the elderly.

AMERICAN JOURNAL OF NEURORADIOLOGY. Interobserver Agreement after Pipeline Embolization Device Implantation

S.H. Suh, H.J. Cloft, G. Lanzino, K. Woodward and D.F. Kallmes

BACKGROUND AND PURPOSE: Although flow diversion devices are popular in treatment of aneurysms, angiographic assessment with these devices has rarely been verified by interobserver variability study. The purpose of this study was to determine the interobserver agreement of a 3-point grading system for assessing the angiographic outcome after flow diversion therapy of intracranial, saccular aneurysms and to determine factors affecting such agreement.

AMERICAN JOURNAL OF NEURORADIOLOGY. Effect of Endovascular Treatment on Headache in Elderly Patients with Unruptured Intracranial Aneurysms

D.-Q. Gu, C.-Z. Duan, X.-F. Li, X.-Y. He, L.-F. Lai and S.-X. Su

BACKGROUND AND PURPOSE: The incidence of unruptured intracranial aneurysms is increasing in the elderly population as life expectancy increases, and patients often present with headache. The goal of this study was to determine the effect of endovascular treatment on headache and identify factors associated with headache outcome in elderly patients with unruptured intracranial aneurysms.
