AMERICAN JOURNAL OF NEURORADIOLOGY. Endovascular Management of Distal ACA Aneurysms: Single-Institution Clinical Experience in 22 Consecutive Patients and Literature Review

D.D. Cavalcantia, A.A. Ablaa, N.L. Martirosyana, C.G. McDougalla, R.F. Spetzlera and F.C. Albuquerquea

BACKGROUND AND PURPOSE: Distal ACA aneurysms remain difficult to treat with endovascular therapy, but improved technology and experience allows for the treatment of some of these aneurysms with excellent results. The purpose of this study was to assess the status of endovascular treatment of distal ACA aneurysms by comparing our results with recent endovascular and microsurgical series of distal ACA aneurysms.

AMERICAN JOURNAL OF ROENTGENOLOGY. Effects of Low-Dose Protocols in Endovascular Treatment of Intracranial Aneurysms: Development of Workflow Task Analysis During Cerebral Endovascular Procedures

Jean Christophe Gentric, Pierre Jannin, Brivael Trelhu, Laurent Riffaud, Hélène Raoult, Jean Christophe Ferré and Jean Yves Gauvrit

OBJECTIVE. Reducing radiation exposure through the use of low-dose protocols during cerebral endovascular procedures is recommended, but evaluation of the impact on the procedure itself is difficult and subjective. A workflow task analysis could provide an objective comparison of two different radiation exposure protocols.

THE LANCET NEUROLOGY. Effect of thrombolysis with alteplase within 6 h of acute ischaemic stroke on long-term outcomes (the third International Stroke Trial [IST-3]): 18-month follow-up of a randomised controlled trial

The IST-3 collaborative group

Background: Few data are available from randomised trials about the effect of thrombolysis with alteplase on long-term functional outcome in patients who have had acute ischaemic stroke and no trial has reported effects on health-related quality of life. A secondary objective of the third International Stroke Trial (IST-3) was to assess the effect of thrombolysis on such outcomes at 18 months.

RADIOLOGY. Neuronal Excitotoxicity after Carotid Angioplasty and Stent Placement Procedures

Florentino Nombela, MD, PhD, Miguel Blanco, MD, PhD, Natalia Pérez de la Ossa, MD, PhD, José Luis Caniego, MD, PhD, Tomás Sobrino, PhD, Domingo Escudero, MD, PhD, Francisco Campos, PhD1, María Ángeles Moro, PhD, Ignacio Lizasoaín, MD, PhD, Antoni Dávalos, MD, PhD, José Castillo, MD, PhD and José Vivancos, MD, PhD

Purpose: To investigate if glutamate levels also increase in carotid angioplasty and stent placement (CAS) procedures, since high plasma glutamate levels are associated with ischemic infarction and transient ischemic attacks, but the length of ischemia needed to elicit such elevation has not been assessed.

CARDIOVASCULAR AND INTERVENTIONAL RADIOLOGY. Comparison Between Proximal Versus Distal Protection Devices in 287 Cases of Carotid Revascularization Using Angioplasty and Stenting: Periprocedure Complications, Morbidity, and Mortality

Francisco Hernández-Fernández, G. Parrilla, B. García-Villalba, M. Espinosa de Rueda, J. Zamarro, M. Garrote, A. Moreno

Background: Embolic protection devices may decrease periprocedural thromboembolic complications during carotid angioplasty and stenting (CAS). When using proximal-protection devices (PPDs), protection starts before crossing the lesion. However, in the medical literature, its use is scarcely reported compared with that of distal-protection filters (DPDs). The objective of this study was to compare periprocedure complications, morbidity, and mortality among 287 consecutive cases of CAS performed with PPDs or DPDs.

STROKE. Costs and Cost-Effectiveness of Carotid Stenting Versus Endarterectomy for Patients at Standard Surgical Risk. Results From the Carotid Revascularization Endarterectomy Versus Stenting Trial (CREST)

Katherine R. Vilain, MPH*; Elizabeth A. Magnuson, ScD*; Haiyan Li, MA; Wayne M. Clark, MD; Richard J. Begg, MD; Albert D. Sam II, MD; W. Charles Sternbergh III, MD; Fred A. Weaver, MD, MMM; William A. Gray, MD; Jenifer H. Voeks, PhD; Thomas G. Brott, MD; David J. Cohen, MD, MSc on behalf of the CREST Investigators

Background and Purpose—The Carotid Revascularization Endarterectomy versus Stenting Trial (CREST) demonstrated similar rates of the primary composite end point between carotid artery stenting (CAS) and carotid endarterectomy (CEA), although the risk of stroke was higher with CAS, and the risk of myocardial infarction was higher with CEA. Given the large number of patients who are candidates for these procedures, an understanding of their relative cost and cost-effectiveness may have important implications for health care policy and treatment guidelines.

AMERICAN JOURNAL OF NEURORADIOLOGY. Effect of Core Laboratory and Multiple-Reader Interpretation of Angiographic Images on Follow-Up Outcomes of Coiled Cerebral Aneurysms: A Systematic Review and Meta-Analysis

I. Rezeka, G. Mousand, Z. Wangb, M.H. Muradb and D.F. Kallmesa,c

BACKGROUND AND PURPOSE: Reported rates of recanalization following coil embolization vary widely across studies. Some confounders are known to affect outcomes but others remain questionable. In the current study, we assess differences in reported angiographic outcomes for cerebral aneurysms treated with coil embolization as a function of single vs multiple readers and site investigator vs core laboratory settings.

AMERICAN JOURNAL OF NEURORADIOLOGY. Observer Variability of an Angiographic Grading Scale Used for the Assessment of Intracranial Aneurysms Treated with Flow-Diverting Stents

BACKGROUND AND PURPOSE: Novel angiographic grading scales for the assessment of intracranial aneurysms treated with flow-diverting stents have been recently developed because previous angiographic grading scales cannot be applied to these aneurysms. The purpose of this study was to evaluate the inter- and intraobserver variability of the novel O Kelly Marotta grading scale, which was developed specifically for the angiographic assessment of aneurysms treated with flow-diverting stents.
