NEUROSURGERY. Safety and Validity of Mechanical Thrombectomy and Thrombolysis on Severe Cerebral Venous Sinus Thrombosis

Li, Guangwen MD*,‡; Zeng, Xianwei MD*,§; Hussain, Mohammed PhD*,‖; Meng, Ran MD‡,¶; Liu, Yi PhD‡; Yuan, Kevin PhD‖; Sikharam, Chaitanya PhD‖; Ding, Yuchuan PhD‖; Ling, Feng MD‡; Ji, Xunming MD, PhD‡

Background: Although the majority of patients with cerebral venous sinus thrombosis (CVST) obtain an optimistic clinical outcome after heparin or warfarin treatment, there remains a subgroup of patients who do not respond to conventional anticoagulation treatment. These patients, especially younger people, as documented by hospital-based studies, have a high morbidity and mortality rate.

NEUROSURGERY. Mechanisms of Stroke After Intracranial Angioplasty and Stenting in the SAMMPRIS Trial

Derdeyn, Colin P. MD*; Fiorella, David MD, PhD‡; Lynn, Michael J. MS§; Rumboldt, Zoran MD‖; Cloft, Harry J. MD, PhD¶; Gibson, Daniel MD*; Turan, Tanya N. MD#; Lane, Bethany F. RN§; Janis, L. Scott PhD**; Chimowitz, Marc I. MB, ChB#; for the Stenting and Aggressive Medical Management for Preventing Recurrent Stroke in Intracranial Stenosis Trial Investigators

Background: Enrollment in the Stenting and Aggressive Medical Management for the Prevention of stroke in Intracranial Stenosis (SAMMPRIS) trial was halted owing to higher-than-expected 30-day stroke rates in the stenting arm. Improvement in periprocedural stroke rates from angioplasty and stenting for intracranial atherosclerotic disease (ICAD) requires an understanding of the mechanisms of these events.

NEUROSURGERY. The Relationship Between Delayed Infarcts and Angiographic Vasospasm After Aneurysmal Subarachnoid Hemorrhage

Brown, Robert J. MD; Kumar, Abhay MD; Dhar, Rajat MD; Sampson, Tomoko R. MD; Diringer, Michael N. MD

Background: Delayed cerebral ischemia is common after aneurysmal subarachnoid hemorrhage (aSAH) and is a major contributor to poor outcome. Yet, although generally attributed to arterial vasospasm, neurological deterioration may also occur in the absence of vasospasm.

CATHETERIZATION AND CARDIOVASCULAR INTERVENTIONS. FiberNet—A new embolic protection device for carotid artery stenting

C. Bauer MD, J. Franke MD, S.C. Bertog MD, V. Woerner MD, S. Ghasemzadeh-Asl MD and H. Sievert MD

Background: Though distal filter protection during carotid stenting reduces the risk of cerebrovascular events, periprocedural stroke remains a risk despite their broad usage. This observation may be related to the pore size of common filters. The FiberNet distal filter system is unique by its very small pore size (40 µm) as well as its low profile and flexibility. Little data is available regarding the clinical performance and safety of this device.

RADIOLOGY. Time-resolved Spin-labeled MR Angiography for the Depiction of Cerebral Arteriovenous Malformations: A Comparison of Techniques

Hélène Raoult, MD, Elise Bannier, PhD, Benjamin Robert, PhD, Christian Barillot, PhD, Peter Schmitt, PhD, Jean-Yves Gauvrit, MD, PhD

To assess time-resolved spin-labeled (SL) magnetic resonance (MR) angiographic imaging with a large acquisition time window over two cardiac cycles for characterization of cerebral arteriovenous malformations (AVMs).

CARDIOVASCULAR AND INTERVENTIONAL RADIOLOGY. Ethanolamine oleate sclerotherapy combined with transarterial embolization using n-butyl cyanoacrylate for extracranial arteriovenous malformations

Akira Kitagawa, Yuichiro Izumi, Makiyo Hagihara, Seiji Kamei, Shuji Ikeda, Eisuke Katsuda, Junko Kimura, Toyohiro Ota, Tsuneo Ishiguchi

Purpose: This study was designed to assess the safety and effectiveness of ethanolamine oleate (EO) sclerotherapy combined with transarterial embolization using a liquid adhesive agent (n-butyl cyanoacrylate, NBCA) for treatment of extracranial arteriovenous malformations (AVMs).

AMERICAN JOURNAL OF ROENTGENOLOGY. MRI and MR Angiography Findings to Differentiate Jugular Venous Reflux From Cavernous Dural Arteriovenous Fistula

Eunhee Kim, Jae Hyoung Kim, Byung Se Choi, Cheolkyu Jung and Dong Hoon Lee

OBJECTIVE. Both jugular venous reflux (JVR) and cavernous dural arteriovenous fistula (DAVF) manifest as abnormal venous signal intensities on time-of-flight (TOF) MR angiography (MRA). We investigated brain MRI and MRA findings that might differentiate JVR from cavernous DAVF.

RADIOLOGY. Intracranial Dural Arteriovenous Fistulas: Evaluation with 3-T Four-dimensional MR Angiography Using Arterial Spin Labeling

Yasuhiko Iryo, MD, Toshinori Hirai, MD, Yutaka Kai, MD, Masanobu Nakamura, RT, Yoshinori Shigematsu, MD, Mika Kitajima, MD, Minako Azuma, MD, Masanori Komi, RT, Kosuke Morita, RT, Yasuyuki Yamashita, MD

Purpose: To evaluate whether 3-T four-dimensional (4D) arterial spin-labeling (ASL)–based magnetic resonance (MR) angiography is useful for the evaluation of shunt lesions in patients with intracranial dural arteriovenous fistulas (AVFs).

STROKE. Clinical Sciences. Urgent Carotid Surgery and Stenting May Be Safe After Systemic Thrombolysis for Stroke

Linn Koraen-Smith, MD; Thomas Troëng, MD, PhD; Martin Björck, MD, PhD; Björn Kragsterman, MD, PhD; Carl-Magnus Wahlgren, MD, PhD; on behalf of the Swedish Vascular Registry and the Riks-Stroke Collaboration

Background and Purpose: Early carotid surgery or stenting after thrombolytic treatment for stroke has become more common during recent years. It is unclear whether this carries an increased risk of postoperative complications and death. The aim of this nationwide population-based study was, therefore, to investigate the safety of urgently performed carotid procedures in patients treated with thrombolysis for stroke.

STROKE. Clinical Sciences. Plaque Inflammation and Unstable Morphology Are Associated With Early Stroke Recurrence in Symptomatic Carotid Stenosis

Michael Marnane, PhD; Susan Prendeville, MB; Ciaran McDonnell, MD; Imelda Noone, ANP; Mary Barry, MCh; Morgan Crowe, MB; Niall Mulligan, MB; Peter J. Kelly, MD

Background and Purpose: Although symptomatic carotid stenosis is associated with 3-fold increased risk of early stroke recurrence, the pathophysiologic mechanisms of high early stroke risk have not been established. We aimed to investigate the relationship between early stroke recurrence after initial symptoms and histological features of plaque inflammation and instability in resected carotid plaque.
