THE LANCET NEUROLOGY. Time to angiographic reperfusion and clinical outcome after acute ischaemic stroke: an analysis of data from the Interventional Management of Stroke (IMS III) phase 3 trial

Prof Pooja Khatri MD, Sharon D Yeatts PhD b, Mikael Mazighi MD c, Prof Joseph P Broderick MD a, Prof David S Liebeskind MD d, Prof Andrew M Demchuk MD e, Prof Pierre Amarenco MD c, Janice Carrozzella MSN a, Judith Spilker BSN a, Lydia D Foster PhD b, Prof Mayank Goyal MD e, Prof Michael D Hill MD e, Prof Yuko Y Palesch PhD b, Prof Edward C Jauch MD b, Prof E Clarke Haley MD f, Achala Vagal MD a, Prof Thomas A Tomsick MD a, for the IMS III Trialists

Background: The IMS III trial did not show a clinical benefit of endovascular treatment compared with intravenous alteplase (recombinant tissue plasminogen activator) alone for moderate or severe ischaemic strokes. Late reperfusion of tissue that was no longer salvageable could be one explanation, as suggested by previous exploratory studies that showed an association between time to reperfusion and good clinical outcome. We sought to validate this association in a preplanned analysis of data from the IMS III trial.

CATHETERIZATION AND CARDIOVASCULAR INTERVENTIONS. The SYNTAX score does not predict presence of carotid disease in a multivessel coronary disease population

Luca Costanzo MD*, Maria Barbara Campisano MD, Davide Capodanno MD, PhD, Andrea Sole MD, Carmelo Grasso MD, Mario Ragusa MD, Giuseppe Ronsivalle MD, Claudia Tamburino MD, Corrado Tamburino MD, PhD andLuigi Di Pino MD, PhD

Background: Numerous reports have shown the relationship between carotid artery atherosclerosis and coronary artery disease (CAD). However, the association between complex CAD evaluated by SYNTAX score (SxScore) and prevalence of carotid lesion (CL) has not been fully investigated. We sought to assess the prevalence of carotid atherosclerosis in patients with multivessel CAD assessed by SxScore and the relationship between SxScore severity and features of carotid atherosclerosis.

JOURNAL OF NEUROSURGERY. Early experience with stent retrievers and comparison with previous-generation mechanical thrombectomy devices for acute ischemic stroke

Eric M. Deshaies, M.D.1, Amit Singla, M.B.B.S.1, Mark R. Villwock, M.S.1, David J. Padalino, M.D.1, Sameer Sharma, M.B.B.S.2, and Amar Swarnkar, M.B.B.S.3

Object: There is limited information regarding patient outcomes following interventions for stroke during the window for endovascular therapy. Studies have suggested that recently approved stent retrievers are safer and more effective than earlier-generation thrombectomy devices. The authors compared cases in which the Solitaire-FR device was used to those in which a MERCI or Penumbra device was used.

CARDIOVASCULAR AND INTERVENTIONAL RADIOLOGY. Comparison Between Proximal Versus Distal Protection Devices in 287 Cases of Carotid Revascularization Using Angioplasty and Stenting: Periprocedure Complications, Morbidity, and Mortality

Francisco Hernández-Fernández, G. Parrilla, B. García-Villalba, M. Espinosa de Rueda, J. Zamarro, M. Garrote, A. Moreno

Background: Embolic protection devices may decrease periprocedural thromboembolic complications during carotid angioplasty and stenting (CAS). When using proximal-protection devices (PPDs), protection starts before crossing the lesion. However, in the medical literature, its use is scarcely reported compared with that of distal-protection filters (DPDs). The objective of this study was to compare periprocedure complications, morbidity, and mortality among 287 consecutive cases of CAS performed with PPDs or DPDs.
