STROKE. Clinical Sciences. Widening and High Inclination of the Middle Cerebral Artery Bifurcation Are Associated With Presence of Aneurysms

Merih I. Baharoglu, MD; Alexandra Lauric, PhD; Mina G. Safain, MD; James Hippelheuser, BS; Chengyuan Wu, MD; Adel M. Malek, MD, PhD

Background and Purpose: The middle cerebral artery (MCA) bifurcation is a preferred site for aneurysm formation. Wider bifurcation angles have been correlated with increased risk of aneurysm formation. We hypothesized a link between the presence of MCA aneurysms and the angle morphology of the bifurcation.

STROKE. Clinical Sciences. Computed Tomographic Angiography and Cerebral Blood Volume Can Predict Final Infarct Volume and Outcome After Recanalization

Cheemun Lum, MD, FRCP; Muhammad Ejaz Ahmed, MBBS; Satya Patro, MBBS; Rebecca Thornhill, PhD; Matthew Hogan, MD, FRCP; Daniela Iancu, MD; Howard Lesiuk, MD, FRCS; Marlise dos Santos, MD, MSc; Dar Dowlatshahi, MD, PhD, FRCPC; on behalf of the Ottawa Stroke Research Group (OSRG)

Background and Purpose: Recanalization rates are higher in acute anterior stroke treated with stent-retrievers when compared with older techniques. However, some still have sizeable infarcts and poor outcome. This may be related to underestimation of core infarct on nonenhanced computed tomography (NECT). CT angiography (CTA) source images (CTASI) and CT perfusion may be more informative. We hypothesize that core infarct estimation with NECT, CTA, and CT perfusion predicts infarct at 24 hours and outcome after fast recanalization.

STROKE. Clinical Sciences. Three-Dimensional Carotid Ultrasound Plaque Texture Predicts Vascular Events

Arna van Engelen, PhD; Thapat Wannarong, MD; Grace Parraga, PhD; Wiro J. Niessen, PhD; Aaron Fenster, PhD; J. David Spence, MD; Marleen de Bruijne, PhD

Background and Purpose: Carotid ultrasound atherosclerosis measurements, including those of the arterial wall and plaque, provide a way to monitor patients at risk of vascular events. Our objective was to examine carotid ultrasound plaque texture measurements and the change in carotid plaque texture during 1 year in patients at risk of events and to compare these with measurements of plaque volume and other risk factors as predictors of vascular events.

STROKE. Clinical Sciences. Defining Clinically Relevant Cerebral Hemorrhage After Thrombolytic Therapy for Stroke. Analysis of the National Institute of Neurological Disorders and Stroke Tissue-Type Plasminogen Activator Trials

Neal M. Rao, MD; Steven R. Levine, MD; Jeffrey A. Gornbein, DrPH; Jeffrey L. Saver, MD

Background and Purpose: Several definitions have been proposed to distinguish clinically relevant from incidental cerebral hemorrhagic transformation after thrombolytic therapy for acute ischemic stroke. We investigated which definition best identifies cerebral hemorrhages that alter long-term functional outcome in the National Institute of Neurological Disorders and Stroke (NINDS) tissue-type plasminogen activator (tPA) trials.

NEUROSURGERY. Feasibility and Safety of Pipeline Embolization Device in Patients With Ruptured Carotid Blister Aneurysms

Yoon, Jang W. MD, MS*; Siddiqui, Adnan H. MD‡; Dumont, Travis M. MD‡,§; Levy, Elad I. MD‡; Hopkins, L. Nelson MD‡; Lanzino, Giuseppe MD¶; Lopes, Demetrius K. MD‖; Moftakhar, Roham MD‖; Billingsley, Joshua T. MD‖; Welch, Babu G. MD#; Boulos, Alan S. MD**; Yamamoto, Junichi MD, PhD**; Tawk, Rabih G. MD*; Ringer, Andrew J. MD‡‡; Hanel, Ricardo A. MD, PhD*,§§; for the Endovascular Neurosurgery Research Group

Background: Treatment of internal carotid ruptured blister aneurysms (IC-RBA) presents many challenges to neurosurgeons because of the high propensity for rebleeding during intervention. The role of a Pipeline Embolization Device (PED) in the treatment of this challenging aneurysm subtype remains undefined despite theoretical advantages.

STROKE. Clinical Sciences. Coated-Platelets Improve Prediction of Stroke and Transient Ischemic Attack in Asymptomatic Internal Carotid Artery Stenosis

Angelia C. Kirkpatrick, MD, MPH; Alfonso J. Tafur, MD, MS; Andrea S. Vincent, PhD; George L. Dale, PhD; Calin I. Prodan, MD

Background and Purpose: Coated-platelets, a subset of procoagulant platelets observed on dual agonist stimulation with collagen and thrombin, support a robust prothrombinase activity and provide a unique measure of platelet thrombotic potential. Coated-platelet levels are increased in large artery stroke, and higher levels are associated with early stroke recurrence, suggesting a potential role for risk stratification in asymptomatic patients with carotid artery stenosis.

STROKE. Contrast-Enhanced Ultrasound for the Evaluation of Neovascularization in Atherosclerotic Carotid Artery Plaques

Kozue Saito, MD; Kazuyuki Nagatsuka, MD; Hatsue Ishibashi-Ueda, MD; Akihiro Watanabe, MD; Hideaki Kannki, MD; Koji Iihara, MD

Background and Purpose: Neovascularization associated with plaque vulnerability, particularly in the plaque shoulder, is susceptible to rupture, causing ischemic events. We aimed to use contrast-enhanced ultrasound to evaluate neovessels in carotid plaques quantitatively, focusing on plaque shoulders.
