Brain Aneurysm Guidelines Emphasize High-Volume Centers

An update to the American Heart Association and American Stroke Association guidelines for the management of aneurysmal subarachnoid hemorrhage places new emphasis on treating patients at high-volume centers with multidisciplinary teams and following up with patients after treatment to detect rebleeding and delayed cerebral ischemia, along with other potential complications.

Make the Most of 2013 Stroke Metrics Requirements

Stroke is one of the new measures that hospitals must report on starting Jan. 1, 2013, to receive full Medicare reimbursement for stroke care delivered in 2015. The requirement is part of the Centers for Medicare and Medicaid Services (CMS) Hospital Inpatient Quality Reporting Program, which was established in 2003, with the goal of improving inpatient care.

Short Sleep May Signal Stroke Risk

People who habitually sleep less than 6 hours appear to be at risk of developing symptoms that may predict future stroke, researchers reported here at the annual meeting of the Associated Professional Sleep Societies. Study participants with normal body mass indices (BMIs) who slept less than 6 hours a night had a 4.5-fold increased risk of having a stroke-like symptom compared with similar individuals who got 8 to 9 hours of sleep a night (P=0.002), Megan Ruiter, PhD, a postdoctoral fellow at the University of Alabama at Birmingham, said in an oral presentation.

Mental Distress Linked to Stroke Mortality

Individuals scoring higher on a measure of general mental health had an increased risk of stroke mortality, a population-based study showed. Psychological distress predicted a significant 66% elevated risk of death from cerebrovascular disease, Mark Hamer, PhD, of the University College London, and colleagues found.

TPA Safe in Stroke Patients on Warfarin

Patients who were taking warfarin were no more likely to develop symptomatic intracranial hemorrhage after treatment with intravenous tissue plasminogen activator (tPA) than those not on the anticoagulant, a large observational study showed. Nor was there an increased risk of serious systemic hemorrhage, any tPA complications, or in-hospital mortality, according to Eric Peterson, MD, of the Duke Clinical Research Institute, and colleagues.

Hormone May Aid Stroke Recovery

Stroke survivors had significantly better functional and cognitive outcomes when they received a nitric oxide-inducing hormone in addition to rehabilitation, Italian investigators reported. At 20 days, patients who received porcine-derived relaxin showed significantly better recovery of global and cognitive function (P=0.001 and P=0.002, respectively), but not daily activities, as compared with patients randomized to rehabilitation alone.

Statins Equally Effective in Men and Women

A simple risk prediction model that uses clinical and echocardiographic characteristics may help identify ischemic stroke patients at a higher risk of future adverse outcomes, researchers found. Age, chronic renal failure, and the amount of calcification around the aortic root were predictive of death at about 4 years after nonhemorrhagic stroke, Avinash Murthy, MD, from the Albany Medical Center in Albany, N.Y., and colleagues reported.

A randomised controlled trial published in The Lancet Neurology in May has shown that for patients with suspected stroke, treatment in a mobile stroke unit substantially reduced median time from call for help to therapy decision. However, there was no substantial difference in the number of patients who received intravenous thrombolysis, or in neurological outcome.
