STROKE. Effect of Time to Reperfusion on Clinical Outcome of Anterior Circulation Strokes Treated With Thrombectomy. Pooled Analysis of the MERCI and Multi MERCI Trials

Raul G. Nogueira, MD; Wade S. Smith, MD, PhD; Gene Sung, MD; Gary Duckwiler, MD; Gary Walker, PhD; Robin Roberts, BSc, MSc; Jeffrey L. Saver, MD; David S. Liebeskind, MD on Behalf of the MERCI and Multi MERCI Writing Committee

Background and Purpose—Previous studies have demonstrated a strong correlation between treatment time and outcomes after intravenous recombinant tissue-type plasminogen activator. However, the temporal profile of ischemia may vary according to the level of occlusion, and it is likely that more proximal occlusions have a more variable temporal course than their distal counterparts. We sought to establish how time influences outcomes in anterior circulation proximal arterial occlusions. STROKE. Prevalence and Prognosis of Asymptomatic Vertebral Artery Origin Stenosis in Patients With Clinically Manifest Arterial Disease

Annette Compter, MD; H. Bart van der Worp, MD; Ale Algra, MD; L. Jaap Kappelle, MD on behalf of the Second Manifestations of ARTerial disease (SMART) Study Group

Background and Purpose—The risk of ischemic stroke in patients with asymptomatic vertebral artery stenosis is unknown. NEUROSURGERY. Radiosurgery to the Surgical Cavity as Adjuvant Therapy for Resected Brain Metastasis

Robbins, Jared R. MD; Ryu, Samuel MD; Kalkanis, Steven MD; Cogan, Chad MS; Rock, Jack MD; Movsas, Benjamin MD; Kim, Jae Ho MD; Rosenblum, Mark MD

BACKGROUND: The standard treatment of resected brain metastasis is whole-brain radiotherapy (WBRT). To avoid the potential toxicity of WBRT and to improve local control, we have used radiosurgery alone to the surgical cavity. OBJECTIVE: To demonstrate the rates of local control, new intracranial metastasis, and overall survival using this treatment scheme without WBRT. NEURORADIOLOGY. Endovascular treatment of proximal superior middle cerebral artery aneurysms

Boris Lubicz, Martina Pezzullo, Denis Brisbois, Olivier De Witte and Benjamin Mine

Introduction: Aneurysms superiorly located on the proximal segment of the middle cerebral artery (PMCAA) are rare and challenging to treat. No information is available regarding their management by endovascular approach. The aim of this study was to report our experience with endovascular treatment (EVT) of PMCAA. STROKE. High-Intensity Signal on Time-of-Flight Magnetic Resonance Angiography Indicates Carotid Plaques at High Risk for Cerebral Embolism During Stenting

Shinichi Yoshimura, MD, PhD*; Kiyofumi Yamada, MD*; Masanori Kawasaki, MD, PhD; Takahiko Asano, MD; Masayuki Kanematsu, MD; Manabu Takamatsu, MD; Akira Hara , MD, PhD; Toru Iwama, MD, PhD

Background and Purpose—A major disadvantage of carotid artery stenting (CAS) compared to carotid endarterectomy is the increased risk of cerebral embolism. Thus, establishing a simple method to discriminate fragile plaques on preoperative routine examination is important. The present study examined whether high-intensity signal (HIS) in the plaque on time-of-flight (TOF) MRA, performed for screening, can discriminate plaque at high risk for cerebral embolism during CAS. 2011; 42: 3132-3137 Published online before print August 25, 2011, doi: 10.1161/STROKEAHA.111.615708. NEUROSURGERY. Ruptured Intracranial Aneurysms With Small Basal Outpouching: Incidence of Basal Rupture and Results of Surgical and Endovascular Treatments

Park, Jaechan MD; Woo, Hyunjin MD; Kang, Dong-Hun MD; Kim, Yongsun MD, PhD; Baik, Seung Kug MD, PhD

BACKGROUND: Recognizing an aneurysmal basal rupture using angiographic evaluation is crucial for optimal treatment. OBJECTIVE: To evaluate the incidence of a small basal outpouching (the most common angiographic configuration suggesting a basal rupture), the incidence of a ruptured basal outpouching, and the results of surgical and endovascular treatments. STROKE. Intracranial Hemorrhage, Outcome, and Mortality After Intra-Arterial Therapy for Acute Ischemic Stroke in Patients Under Oral Anticoagulants

Gian Marco De Marchis, MD*; Simon Jung, MD*; Giuseppe Colucci, MD; Niklaus Meier, MD; Urs Fischer, MD; Anja Weck, MD; Marie-Luise Mono, MD; Aekaterini Galimanis, MD; Heinrich P. Mattle, MD; Gerhard Schroth, MD; Jan Gralla, MD; Marcel Arnold, MD; Caspar Brekenfeld, MD

Background and Purpose—Use of intravenous tissue-type plasminogen activator (IV tPA) for acute ischemic stroke is restricted to patients with an international normalized ratio (INR) less than 1.7. However, a recent study showed increased risk of symptomatic intracranial hemorrhage after IV tPA use in patients with oral anticoagulants (OAC) even with an INR less than 1.7. The present study assessed the risk of symptomatic intracranial hemorrhage, clinical outcome, and mortality after intra-arterial therapy (IAT) in patients with and without previous use of OAC. STROKE. Plaque Distribution of Stenotic Middle Cerebral Artery and Its Clinical Relevance

Wei-Hai Xu, MD*; Ming-Li Li, MD*; Shan Gao, MD; Jun Ni, MD; Li-Xin Zhou, MD; Ming Yao, MD; Bin Peng, MD; Feng Feng, MD; Zheng-Yu Jin, MD; Li-Ying Cui, MD

Background and Purpose—Microanatomy studies reveal that most penetrating branches of middle cerebral artery (MCA) arise from the dorsal–superior surface of the trunk. Using high-resolution MRI, we sought to explore the plaque distribution of MCA atherosclerosis and its clinical relevance in relation to the orifices of penetrating arteries.
