AMERICAN JOURNAL OF NEURORADIOLOGY. An In Vivo, MRI-Integrated Real-Time Model of Active Contrast Extravasation in Acute Intracerebral Hemorrhage

R.I. Aviv, T. Huynh, Y. Huang, D. Ramsay, P. Van Slyke, D. Dumont, P. Asmah, R. Alkins, R. Liu, and K. Hynynen

Background and Purpose: The “spot sign” or contrast extravasation is strongly associated with hematoma formation and growth. An animal model of contrast extravasation is important to test existing and novel therapeutic interventions to inform present and future clinical studies. The purpose of this study was to create an animal model of contrast extravasation in acute intracerebral hemorrhage.

AMERICAN JOURNAL OF NEURORADIOLOGY. Diffusion-Weighted Imaging with Dual-Echo Echo-Planar Imaging for Better Sensitivity to Acute Stroke

S.J. Holdsworth, K.W. Yeom, M.U. Antonucci, J.B. Andre, J. Rosenberg, M. Aksoy, M. Straka, N.J. Fischbein, R. Bammer, M.E. Moseley, G. Zaharchuk, and S. Skare

BACKGROUND AND PURPOSE: Parallel imaging facilitates the acquisition of echo-planar images with a reduced TE, enabling the incorporation of an additional image at a later TE. Here we investigated the use of a parallel imaging–enhanced dual-echo EPI sequence to improve lesion conspicuity in diffusion-weighted imaging.

AMERICAN JOURNAL OF ROENTGENOLOGY. Comparison of Enterprise With Neuroform Stent-Assisted Coiling of Intracranial Aneurysms

Yasha Kadkhodayan, Nicholas Rhodes, Spiros Blackburn, Colin P. Derdeyn, DeWitte T. Cross, III and Christopher J. Moran

OBJECTIVE. The Enterprise stent is the first closed-cell stent designed to treat wide-necked intracranial aneurysms. Advantages of the design can include improvement in keeping coils within an aneurysm and the ability of the stent to be recaptured. We compared the technical and clinical complications of the Enterprise stent with the open-cell Neuroform stent, its primary alternative.

AMERICAN JOURNAL OF NEURORADIOLOGY. Mind the Gap: Impact of Computational Fluid Dynamics Solution Strategy on Prediction of Intracranial Aneurysm Hemodynamics and Rupture Status Indicators

K. Valen-Sendstad and D.A. Steinman

Background and Purpose: Computational fluid dynamics has become a popular tool for studying intracranial aneurysm hemodynamics, demonstrating success for retrospectively discriminating rupture status; however, recent highly refined simulations suggest potential deficiencies in solution strategies normally used in the aneurysm computational fluid dynamics literature. The purpose of the present study was to determine the impact of this gap.

SPRINGER. Quantitative comparison of the dynamic flow waveform changes in 12 ruptured and 29 unruptured ICA–ophthalmic artery aneurysms

Aichi Chien, James Sayre, Fernando Viñuela

Introduction: Studies have reported a correlation between blood flow dynamics in the cardiac cycle and vascular diseases, but research to analyze the dynamic changes of flow in cerebral aneurysms is limited. This quantitative study investigates the temporal changes in flow during a cardiac cycle (flow waveform) in different regions of aneurysms and their association with aneurysm rupture.

NEUROSURGERY. The Neurological Emergency Room and Prehospital Stroke Alert: The Whole Is Greater Than the Sum of Its Parts

Background: Emergency medical services (EMS) prenotification to hospitals regarding the arrival of patients who have had a stroke is recommended to facilitate the workup once the patient arrives. Most hospitals have the patient enter the emergency department (ED) before obtaining a head computed tomography (CT) scan. At Capital Health, prehospital stroke-alert patients are delivered directly to CT and met by a neurological emergency team. The goal of bypassing the ED is to reduce the time to treatment.

NEUROSURGERY. Percutaneous Stereotactic Radiofrequency Lesioning for Trigeminal Neuralgia: Determination of Minimum Clinically Important Difference in Pain Improvement for Patient-Reported Outcomes

Reddy, Vishruth K. BA*; Parker, Scott L. MD*; Lockney, Dennis T. MD*; Patrawala, Samit A. MD*; Su, Pei-Fang PhD‡,§; Mericle, Robert A. MD¶

Background: The Visual Analog Scale (VAS) and the Barrow Neurological Institute Pain Scale (BNI-PS) are 2 patient-reported outcome (PRO) tools frequently used to rate pain from trigeminal neuralgia (TN). Outcomes studies often use these patient-reported outcomes to assess treatment effectiveness, but it is unknown exactly what degree of change in the numerical scores constitutes the minimum clinically important difference (MCID). MCID remains uninvestigated for percutaneous stereotactic radiofrequency lesioning (RFL), a common surgical procedure for TN.

STROKE. Exploring the Age of Intracranial Aneurysms Using Carbon Birth Dating

Nima Etminan, MD; Rita Dreier, PhD; Bruce A. Buchholz, PhD; Peter Bruckner, PhD; Hans-Jakob Steiger, MD; Daniel Hänggi, MD; R. Loch Macdonald, MD, PhD

Background and Purpose: There is a controversy about the time span over which cerebral aneurysms develop. In particular, it is unknown whether collagen in ruptured aneurysms undergoes more rapid turnover than in unruptured aneurysms.14C birth dating of collagen could be used to address this question.

AMERICAN JOURNAL OF NEURORADIOLOGY. Review of 2 Decades of Aneurysm-Recurrence Literature, Part 2: Managing Recurrence after Endovascular Coiling

E. Crobeddu, G. Lanzino, D.F. Kallmes and H.J. Cloft

Summary: Cerebral aneurysms are treated to prevent hemorrhage or rehemorrhage. Angiographic recurrences following endovascular therapy have been a problem since the advent of this treatment technique, even though posttreatment hemorrhage remains rare. Notwithstanding its unclear clinical significance, angiographic recurrence remains not only a prime focus in the literature but also frequently leads to potentially risky retreatments. The literature regarding aneurysm recurrence following endovascular therapy, spanning 2 decades, is immense and immensely confusing. We review the topic of recurrence following endovascular treatment of cerebral aneurysms in an effort to distill it down to fundamental material relevant to clinical practice.
