AMERICAN JOURNAL OF NEURORADIOLOGY. Flow Conditions in the Intracranial Aneurysm Lumen Are Associated with Inflammation and Degenerative Changes of the Aneurysm Wall

J. Cebral, E. Ollikainen, B.J. Chung, F. Mut, V. Sippola, B.R. Jahromi, R. Tulamo, J. Hernesniemi, M. Niemelä, A. Robertson and J. Frösen

PURPOSE: Saccular intracranial aneurysm is a common disease that may cause devastating intracranial hemorrhage. Hemodynamics, wall remodeling, and wall inflammation have been associated with saccular intracranial aneurysm rupture. We investigated how saccular intracranial aneurysm hemodynamics is associated with wall remodeling and inflammation of the saccular intracranial aneurysm wall.

STROKE. Risk of Pulmonary Embolism After Cerebral Venous Thrombosis

Ava L. Liberman, Alexander E. Merkler, Gino Gialdini, Steven R. Messé, Michael P. Lerario, Santosh B. Murthy, Hooman Kamel and Babak B. Navi

Purpose: Cerebral vein thrombosis (CVT) is a type of venous thromboembolism. Whether the risk of pulmonary embolism (PE) after CVT is similar to the risk after deep venous thrombosis (DVT) is unknown.

STROKE. Large Vessel Occlusion Scales Increase Delivery to Endovascular Centers Without Excessive Harm From Misclassifications

Henry Zhao, Skye Coote, Lauren Pesavento, Leonid Churilov, Helen M. Dewey, Stephen M. Davis and Bruce C.V. Campbell

Purpose: Clinical large vessel occlusion (LVO) triage scales were developed to identify and bypass LVO to endovascular centers. However, there are concerns that scale misclassification of patients may cause excessive harm. We studied the settings where misclassifications were likely to occur and the consequences of these misclassifications in a representative stroke population.

STROKE. Simultaneous Multiple Intracerebral Hemorrhages (SMICH)

Teddy Y. Wu, Nawaf Yassi, Darshan G. Shah, Minmin Ma, Gagan Sharma, Jukka Putaala, Daniel Strbian, Bruce C.V. Campbell, Bernard Yan, Turgut Tatlisumak, Patricia M. Desmond, Stephen M. Davis and Atte Meretoja

Purpose: Simultaneous multiple intracerebral hemorrhages (SMICHs) are uncommon. Few single-center studies have analyzed characteristics and outcome of SMICH. We analyzed clinical characteristics and outcome of SMICH patients from 2 comprehensive stroke centers.

STROKE. Ten-Year Temporal Trends in Medical Complications After Acute Intracerebral Hemorrhage in the United States

Fadar Oliver Otite, Priyank Khandelwal, Amer M. Malik, Seemant Chaturvedi, Ralph L. Sacco and Jose G. Romano

Purpose: Data on medical complications after intracerebral hemorrhage (ICH) are sparse. We assessed trends in the prevalence of urinary tract infection, pneumonia, sepsis, deep venous thrombosis (DVT), pulmonary embolism, acute renal failure (ARF), and acute myocardial infarction after ICH in the United States.

STROKE. The Basilar Artery on Computed Tomography Angiography Prognostic Score for Basilar Artery Occlusion

Fana Alemseged, Darshan G. Shah, Marina Diomedi, Fabrizio Sallustio, Andrew Bivard, Gagan Sharma, Peter J. Mitchell, Richard J. Dowling, Steven Bush, Bernard Yan, Carlo Caltagirone, Roberto Floris, Mark W. Parsons, Christopher R. Levi, Stephen M. Davis and Bruce C.V. Campbell

Purpose: Basilar artery occlusion is associated with high risk of disability and mortality. This study aimed to assess the prognostic value of a new radiological score: the Basilar Artery on Computed Tomography Angiography (BATMAN) score.

STROKE. Multiparametric Magnetic Resonance Imaging for Prediction of Parenchymal Hemorrhage in Acute Ischemic Stroke After Reperfusion Therapy

Kambiz Nael, James R. Knitter, Reza Jahan, Jeffery Gornbein, Zahra Ajani, Lei Feng, Brett C. Meyer, Lee H. Schwamm, Albert J. Yoo, Randolph S. Marshall, Philip M. Meyers, Dileep R. Yavagal, Max Wintermark, David S. Liebeskind, Judy Guzy, Sidney Starkman, Jeffrey L. Saver and Chelsea S. Kidwell

Purpose: Patients with acute ischemic stroke are at increased risk of developing parenchymal hemorrhage (PH), particularly in the setting of reperfusion therapies. We have developed a predictive model to examine the risk of PH using combined magnetic resonance perfusion and diffusion parameters, including cerebral blood volume (CBV), apparent diffusion coefficient, and microvascular permeability (K2).

STROKE. Prehospital Scales for Large Vessel Occlusion

Patrik Michel

The strong evidence for benefits of rapid thrombectomy in patients with acute ischemic stroke (AIS) is challenging prehospital systems around the world. Although high-tech endovascular treatment (EVT) works only for a minority of patients, healthcare systems cannot afford to miss suitable candidates anymore. Recent figures show eligibility of =25% for thrombolysis and =10% for EVT in all AIS arriving at the hospital within 24 hours. Still, paramedics, local hospitals, and primary stroke centers are struggling to clinically identify the AIS patients who are likely to have intracranial large vessel occlusions (LVO) in the anterior and posterior circulation.

Purpose: The prediction of genetic predispositions to ischemic stroke (IS) may allow the identification of individuals at elevated risk and thereby prevent IS in clinical practice. Previously developed weighted multilocus genetic risk scores showed limited predictive ability for IS. Here, we investigated the predictive ability of a newer method, polygenic risk score (polyGRS), based on the idea that a few strong signals, as well as several weaker signals, can be collectively informative to determine IS risk.

STROKE. Clinical Selection Strategies to Identify Ischemic Stroke Patients With Large Anterior Vessel Occlusion

Jan F. Scheitz, Azmil H. Abdul-Rahim, Rachael L. MacIsaac, Charith Cooray, Heidi Sucharew, Dawn Kleindorfer, Pooja Khatri, Joseph P. Broderick, Heinrich J. Audebert, Niaz Ahmed, Nils Wahlgren, Matthias Endres, Christian H. Nolte and Kennedy R. Lees

Purpose: The National Institutes of Health Stroke Scale (NIHSS) correlates with presence of large anterior vessel occlusion (LAVO). However, the application of the full NIHSS in the prehospital setting to select patients eligible for treatment with thrombectomy is limited. Therefore, we aimed to evaluate the prognostic value of simple clinical selection strategies.

STROKE. Optimal Timing of Anticoagulant Treatment After Intracerebral Hemorrhage in Patients With Atrial Fibrillation

Johanna Pennlert, Rosanna Overholser, Kjell Asplund, Bo Carlberg, Bart Van Rompaye, Per-Gunnar Wiklund and Marie Eriksson

Purpose: This study aims to provide observational data on the relationship between the timing of antithrombotic treatment and the competing risks of severe thrombotic and hemorrhagic events in a cohort of Swedish patients with atrial fibrillation and intracerebral hemorrhage (ICH).
