Abstract: Tranarterial chemoembolization (TACE) has been established by a meta-analysis of randomized controlled trials as the standard of care for nonsurgical patients with large or multinodular noninvasive hepatocellular carcinoma (HCC) isolated to the liver and with preserved liver function. Although conventional TACE with administration of an anticancer-in-oil emulsion followed by embolic agents has been the most popular technique, the introduction of embolic drug-eluting beads has provided an alternative to lipiodol-based regimens. RADIOLOGY. Percutaneous Ablation in the Kidney

Percutaneous ablation in the kidney is now performed as a standard therapeutic nephron-sparing option in patients who are poor candidates for resection. Its increasing use has been largely prompted by the rising incidental detection of renal cell carcinomas with cross-sectional imaging and the need to preserve renal function in patients with comorbid conditions, multiple renal cell carcinomas, and/or heritable renal cancer syndromes. Clinical studies to date indicate that radiofrequency ablation and cryoablation are effective therapies with acceptable short- to intermediate-term outcomes and with a low risk in the appropriate setting, with attention to pre-, peri-, and postprocedural detail. The results following percutaneous radiofrequency ablation and cryoablation in the treatment of renal cell carcinoma are reviewed in this article, including those of several larger scale studies of ablation of T1a tumors. SEMINARS IN INTERVENTIONAL RADIOLOGY. Transcatheter Arterial Embolization of Hepatic Arteriovenous Shunts in Patients with Hepatocellular Carcinoma

Preprocedure: Most hepatic arteriovenous shunts (HAVS) associated with hepatocellular carcinoma (HCC) occur with advanced stage liver cancer, and these patients typically have a dismal prognosis. Transcatheter arterial embolization (TAE) in this population carries significant risk. Therefore, proper patient selection is critical to determine which patients may benefit from TAE. The Child-Pugh score should be calculated because TAE is poorly tolerated in HCC patients with Child-Pugh class C. We choose transcatheter arterial infusion instead of TAE for those patients. In patients with elevated liver enzymes in addition to increased serum bilirubin levels, there is a propensity for hepatic failure and thus TAE may be contraindicated. Chronic kidney disease (CKD) is frequently observed in patients with arterial hypertension. The same factors that promote the appearance and progression of atherosclerosis can also promote the development of CKD. Two parameters are usually measured to estimate alterations in renal function, the presence of albuminuria, and the estimation of glomerular filtration rate (GFR). Microalbuminuria and a decreased estimated GFR (<60 mL/min/1.73 m2) are both accompanied by a significant increase in cardiovascular (CV) risk. Chronic kidney disease can develop all over the cardiorenal continuum and its presence in hypertensive patients with already developed CV disease contributes to a further increase in the development of events and death. Renal protection will in turn obtain CV protection and the treatment to be used is similar to that employed to prevent or to treat established CV disease. The diagnosis and management of patients with renovascular disease and hypertension continue to elude healthcare providers. The advent of novel imaging and interventional techniques, and increased understanding of the pathways leading to irreversible renal injury and renovascular hypertension, have ushered in commendable attempts to optimize and finetune strategies to preserve or restore renal function and control blood pressure. Large randomized clinical trials that compare different forms of therapy, and smaller trials that test novel experimental treatments, will hopefully help formulate innovative concepts and tools to manage the patient population with atherosclerotic renovascular disease.
