JOURNAL OF VASCULAR SURGERY. In situ laser fenestration during emergent thoracic endovascular aortic repair is an effective method for left subclavian artery revascularization

Richard E. Redlinger Jr., MD , Sadaf S. Ahanchi, MD , Jean M. Panneton, MD

Background: Retrograde laser fenestration of the left subclavian artery (LSA) during emergent thoracic endovascular aortic repair (TEVAR) uses a relatively simple intraoperative method of endograft modification to revascularize aortic branches for a variety of acute thoracic aortic pathologies. This study presents our expanded experience and midterm outcomes of TEVAR with laser fenestration to revascularize the LSA as an alternative to debranching.

CIRCULATION. Drug-Eluting Balloon in Peripheral Intervention for Below the Knee Angioplasty Evaluation (DEBATE-BTK) A Randomized Trial in Diabetic Patients With Critical Limb Ischemia

Francesco Liistro, MD; Italo Porto, MD PhD; Paolo Angioli, MD; Simone Grotti, MD; Lucia Ricci, MD; Kenneth Ducci, MD; Giovanni Falsini, MD; Giorgio Ventoruzzo, MD; Filippo Turini, MD; Guido Bellandi, MD; Leonardo Bolognese, MD

Background—The 1-year restenosis rate after balloon angioplasty of long lesions in below-the-knee arteries may be as high as 70%. Our aim was to investigate the efficacy of a paclitaxel drug-eluting balloons versus conventional percutaneous transluminal angioplasty (PTA) for the reduction of restenosis in diabetic patients with critical limb ischemia undergoing endovascular intervention of below-the-knee arteries.

JOURNAL OF VASCULAR SURGERY. Atherectomy offers no benefits over balloon angioplasty in tibial interventions for critical limb ischemia

Kevin E. Todd Jr., BS , Sadaf S. Ahanchi, MD , Christian A. Maurer, BS , Jung H. Kim, MPH , Candice R. Chipman, MD , Jean M. Panneton, MD

Background: Endovascular adjuncts, like atherectomy, were developed to improve outcomes of endovascular arterial interventions. The true impact of atherectomy on endovascular outcomes remains to be determined, and little data exist on the influence of atherectomy on tibial interventions. Our study compares early and late outcomes of tibial intervention with angioplasty vs atherectomy-assisted interventions.

CIRCULATION. Direct Quantitative Assessment of the Peripheral Artery Collateral Circulation in Patients Undergoing Angiography

Tobias Traupe, MD; Jana Ortmann, MD; Michael Stoller, MD; Iris Baumgartner, MD; Stefano F. de Marchi, MD; Christian Seiler, MD

Background—Despite the fact that numerous studies have pursued the strategy of improving collateral function in patients with peripheral artery disease, there is currently no method available to quantify collateral arterial function of the lower limb.

JOUNAL OF VASCULAR SURGERY. Subclavian revascularization in the age of thoracic endovascular aortic repair and comparison of outcomes in patients with occlusive disease

Salvatore T. Scali, MD , Catherine K. Chang, MD , Stephen G. Pape, BS , Robert J. Feezor, MD , Scott A. Berceli, MD, PhD , Thomas S. Huber, MD, PhD , Adam W. Beck, MD

Objective: Open surgical revascularization for subclavian artery occlusive disease (OD) has largely been supplanted by endovascular treatment despite the excellent long-term patency of bypass. The indications for carotid-subclavian bypass (C-SBP) and subclavian transposition (ST) have been recently expanded with the widespread application of thoracic endovascular aortic repair (TEVAR), primarily to augment proximal landing zones or treat endovascular failures. This study was performed to determine the outcomes of patients undergoing C-SBP/ST in the context of contemporary endovascular therapies and evolving indications.

JOURNAL OF VASCULAR SURGERY. Endovascular repair of aortoiliac aneurysmal disease with the helical iliac bifurcation device and the bifurcated-bifurcated iliac bifurcation device

Shen Wong, MD , Roy K. Greenberg, MD , Chase R. Brown, BS , Tara M. Mastracci, MD , James Bena, MS , Matthew J. Eagleton, MD

Background: Iliac branch device (IBD) treatment of common and internal iliac artery (CIA and IIA) aneurysms has been controversial in the context of available embolization techniques or off-label adjunctive procedures. Two devices exist, a straight IBD (S-IBD) and a helical IBD (H-IBD). We report our midterm results with the latter and present outcomes with a third device intended to treat disease in the presence of short CIAs termed the bifurcated-bifurcated IBD (BB-IBD).
