JOURNAL OF VASCULAR SURGERY. Incidence, predictors, and prognosis of in-stent occlusion after endovascular treatment with nitinol stents for femoropopliteal lesions

Tomoharu Dohi, MD , Osamu Iida, MD , Yoshimitsu Soga, MD , Keisuke Hirano, MD , Kenji Suzuki, MD , Mitsuyoshi Takahara, MD , Masaaki Uematsu, MD, PhD , Shinsuke Nanto, MD, PhD

Objective: Widespread use of self-expanding nitinol stent-based endovascular treatment (EVT) for femoropopliteal (FP) lesions has been fueled by its less-invasive nature and modest durability; however, prevalence, predictors and prognosis of in-stent occlusion are undefined and were investigated here.

JOUNAL OF VASCULAR SURGERY. Late graft explants in endovascular aneurysm repair

Eric J. Turney, MD , Sean P. Steenberge, MS , Sean P. Lyden, MD , Matthew J. Eagleton, MD , Sunita D. Srivastava, MD , Timur P. Sarac, MD , Rebecca L. Kelso, MD , Daniel G. Clair, MD

Objective: With more than a decade of use of endovascular aneurysm repair (EVAR), we expect to see a rise in the number of failing endografts. We review a single-center experience with EVAR explants to identify patterns of presentation and understand operative outcomes that may alter clinical management.

JOUNAL OF VASCULAR SURGERY. Outcome and clinical significance of delayed endoleaks after endovascular aneurysm repair

Wei Zhou, MD , Eddie Blay Jr., BA , Vinit Varu, MD , Syed Ali, BS , Michelle Qiushuang Jin, BA , Lisa Sun, MS , Jin Hyun Joh, MD, PhD

Objective: Endovascular aneurysm repair (EVAR) is considered the standard therapy for most patients with abdominal aortic aneurysm (AAA). Endoleak is a well-known EVAR-related complication that requires long-term follow-up. However, patient follow-up is often challenging outside clinical trials. We sought to evaluate the incidence and the effect of delayed endoleaks in a Veterans Administration (VA) health care system where long-term follow-up is ensured.

JOUNAL OF VASCULAR SURGERY. Safety and feasibility of adjunctive dexamethasone infusion into the adventitia of the femoropopliteal artery following endovascular revascularization

Christopher D. Owens, MD, MSc , Warren J. Gasper, MD , Joy P. Walker, MD , Hugh F. Alley, BA , Michael S. Conte, MD , S. Marlene Grenon, MD

Objective: Restenosis following endovascular treatment of the femoropopliteal segment is associated with the inflammatory response produced in the artery wall at the time of the procedure. Although local drug delivery to the superficial femoral and popliteal arteries promises improved patency, data are currently limited. We hypothesized that improved percutaneous delivery of an anti-inflammatory compound into the adventitia of the femoropopliteal at the time of endovascular treatment would be safe, feasible, and decrease the inflammatory response.

JOUNAL OF VASCULAR SURGERY. Metabolic syndrome is associated with type II endoleak after endovascular abdominal aortic aneurysm repair

Michael R. Hall, MD , Clinton D. Protack, MD , Roland Assi, MD , Willis T. Williams, MD , Daniel J. Wong, BS , Daniel Lu, BS , Bart E. Muhs, MD, PhD , Alan Dardik, MD, PhD

Objective: Type II endoleak is usually a benign finding after endovascular abdominal aortic aneurysm repair (EVAR). In some patients, however, type II endoleak leads to aneurysm sac expansion and the need for further intervention. We examined which factors, in particular the components of metabolic syndrome (MetS), would lead to an increase risk of endoleak after EVAR.

RADIOLOGY. Volumetric Quantification of Type II Endoleaks: An Indicator for Aneurysm Sac Growth Following Endovascular Abdominal Aortic Aneurysm Repair

Shadpour Demehri, MD, Jason Signorelli, MS, Kanako K. Kumamaru, MD, PhD, Nicole Wake, MS, Elizabeth George, MBBS, Michael Hanley, MD1, Michael L. Steinger, MD, Edwin Charles Gravereaux, MD, Frank J. Rybicki, MD, PhD

Purpose: To test the hypothesis that type II endoleak cavity volume (ECV) and endoleak cavity diameter (ECD) measurements are accurate indicators of aneurysm sac volume (ASV) enlargement in patients who undergo endovascular aneurysm repair (EVAR) in the abdominal aorta.

JOURNAL OF VASCULAR SURGERY. Comparative effectiveness of endovascular versus open repair of ruptured abdominal aortic aneurysm in the Medicare population

Samuel T. Edwards, MD , Marc L. Schermerhorn, MD , A. James O Malley, PhD , Rodney P. Bensley, MD , Rob Hurks, MD, PhD , Philip Cotterill, PhD , Bruce E. Landon, MD, MBA, MSc

Objective: Endovascular aortic repair (EVAR) for abdominal aortic aneurysm (AAA) is increasingly used for emergent treatment of ruptured AAA (rAAA). We sought to compare the perioperative and long-term mortality, procedure-related complications, and rates of reintervention of EVAR vs open aortic repair of rAAA in Medicare beneficiaries.

JOURNAL OF VASCULAR SURGERY. Experience with a novel custom-made fenestrated stent graft in the repair of juxtarenal and type IV thoracoabdominal aneurysms

Alexander E. Rolls, MRCS , Michael Jenkins, MS, FRCS , Colin D. Bicknell, MD, FRCS , Celia V. Riga, MD, MRCS , Nick J. Cheshire, MD, FRCS , Nick Burfitt, FRCR , Mohamad Hamady, MD, FRCR

Objective: Fenestrated stent grafting has become increasingly popular as a means to manage complex aortic pathology, including juxta- and pararenal aneurysms. The design of a recently developed custom-made fenestrated stent graft, in theory, confers advantages when managing anatomically challenging aortic morphology. This study evaluated its feasibility in anatomically challenging scenarios.
