JOURNAL OF VASCULAR SURGERY. Cost-effectiveness of endovascular versus open repair of acute complicated type B aortic dissections

Thomas Luebke, MD, PhD , Jan Brunkwall, MD, PhD

Objective: This study weighed the cost and benefit of thoracic endovascular aortic repair (TEVAR) vs open repair (OR) in the treatment of an acute complicated type B aortic dissection by (TBAD) estimating the cost-effectiveness to determine an optimal treatment strategy based on the best currently available evidence.

CATHETERIZATION AND CARDIOVASCULAR INTERVENTIONS. Transpedal access after failed anterograde recanalization of complex below-the-knee and femoropoliteal occlusions in critical limb ischemia

Zoltán Ruzsa MD, PHD1,2,*, Balázs Nemes MD, PHD1, Zoltán Bánsághi MD2,3, Károly Tóth MD2, Ferenc Kuti MD2, Slavka Kudrnova MD1, Balázs Berta MD1, Kálmán Hüttl MD, DSC1 and Béla Merkely MD, DSC

Background: Successful angioplasty is one of the main factor of limb salvage during critical limb ischemia. In complex femoropopliteal to infrapopliteal occlusions, an anterograde recanalization attempt can fail in up to 20% of the cases. The purpose of this dual center pilot study was to evaluate the acute success and clinical impact of retrograde transpedal access for retrograde below-the-knee and femoropopliteal chronic total occlusions after failed anterograde attempt and to access the late complications at the puncture site.

CATHETERIZATION AND CARDIOVASCULAR INTERVENTIONS. Tibio-pedal arterial minimally invasive retrograde revascularization in patients with advanced peripheral vascular disease: The TAMI technique, original case series

J.A. Mustapha MD, Fadi Saab MD, Theresa McGoff BSN, Carmen Heaney BSN, Larry Diaz-Sandoval MD, Matthew Sevensma DO and Barbara Karenko DO

Background: A tibial-pedal access method is needed for patients with advanced peripheral artery disease (PAD) unable to tolerate common femoral artery (CFA) access and intervention due to body habitus or comorbidities. This is the first case series reporting an alternative technique to revascularize such patients. Using ultrasound (US) and the tibio-pedal arterial minimally invasive retrograde revascularization (TAMI) technique, operators accessed, and revascularized the lower extremity completely via tibial-pedal arterial access.

AMERICAN HEART JOURNAL. Association of the ankle-brachial index with history of myocardial infarction and stroke

W. Schuyler Jones, MD , Manesh R. Patel, MD , Caron B. Rockman, MD , Yu Guo, MA , Mark Adelman, MD , Thomas Riles, MD , Jeffrey S. Berger, MD

Background: Ankle-brachial index (ABI) testing is a simple, noninvasive method to diagnose peripheral artery disease (PAD) and is associated with all-cause mortality. The association of ABI levels and myocardial infarction (MI) and stroke is less certain. We sought to further characterize the association between ABI levels and history of MI and stroke.

JACC. Percutaneous Treatment of a Complex Saccular Aortic Pseudoaneurysm With Covered Stenting After Subclavian Artery Translocation

Cecilia Craviari∗; Nathaniel W. Taggart, MD∗; Frank Cetta, MD∗; Donald J. Hagler, MD∗; Thomas C. Bower, MD‡; Daniel A. Mauriello, MD∗; Jonathan N. Johnson, MD∗

A 44-year-old woman presented to our institution for evaluation of an expanding aortic saccular pseudoaneurysm. She had a history of an end-end repair of coarctation of the aorta at 4 years of age. At 29 years of age, she received a diagnosis of a pseudoaneurysm measuring 10 mm at the site of the previous repair. Since that time, the pseudoaneurysm had doubled in size. She also had a history of hypertension and had been treated with atenolol.

RADIOLOGY. Endoleak and In-Stent Thrombus Detection with CT Angiography in a Thoracic Aortic Aneurysm Phantom at Different Tube Energies Using Filtered Back Projection and Iterative Algorithms

Zsuzsanna Deák, MD, Jochen M. Grimm, MD, Fabian Mueck, MD, Lucas L. Geyer, MD, Marcus Treitl, MD, Maximilian F. Reiser, MD, Stefan Wirth, PhD, MD

To determine the lower limit of dose reduction with hybrid and fully iterative reconstruction algorithms in detection of endoleaks and in-stent thrombus of thoracic aorta with computed tomographic (CT) angiography by applying protocols with different tube energies and automated tube current modulation.

THE LANCET. Comparison of the efficacy and safety of new oral anticoagulants with warfarin in patients with atrial fibrillation: a meta-analysis of randomised trials

Dr Christian T Ruff MD, Robert P Giugliano MD , Prof Eugene Braunwald MD, Elaine B Hoffman PhD, Naveen Deenadayalu MPH, Prof Michael D Ezekowitz MBChB, Prof A John Camm MD, Prof Jeffrey I Weitz MD, Prof Basil S Lewis MD, Prof Alexander Parkhomenko MD, Prof Takeshi Yamashita MD, Prof Elliott M Antman MD

Background: our new oral anticoagulants compare favourably with warfarin for stroke prevention in patients with atrial fibrillation; however, the balance between efficacy and safety in subgroups needs better definition. We aimed to assess the relative benefit of new oral anticoagulants in key subgroups, and the effects on important secondary outcomes.

THE LANCET. Metabolic mediators of the effects of body-mass index, overweight, and obesity on coronary heart disease and stroke: a pooled analysis of 97 prospective cohorts with 1·8 million participants

The Global Burden of Metabolic Risk Factors for Chronic Diseases Collaboration (BMI Mediated Effects)

Background: ody-mass index (BMI) and diabetes have increased worldwide, whereas global average blood pressure and cholesterol have decreased or remained unchanged in the past three decades. We quantified how much of the effects of BMI on coronary heart disease and stroke are mediated through blood pressure, cholesterol, and glucose, and how much is independent of these factors. Methods

JOURNAL OF VASCULAR SURGERY. Incidence, predictors, and prognosis of in-stent occlusion after endovascular treatment with nitinol stents for femoropopliteal lesions

Tomoharu Dohi, MD , Osamu Iida, MD , Yoshimitsu Soga, MD , Keisuke Hirano, MD , Kenji Suzuki, MD , Mitsuyoshi Takahara, MD , Masaaki Uematsu, MD, PhD , Shinsuke Nanto, MD, PhD

Objective: Widespread use of self-expanding nitinol stent-based endovascular treatment (EVT) for femoropopliteal (FP) lesions has been fueled by its less-invasive nature and modest durability; however, prevalence, predictors and prognosis of in-stent occlusion are undefined and were investigated here.

JOUNAL OF VASCULAR SURGERY. Late graft explants in endovascular aneurysm repair

Eric J. Turney, MD , Sean P. Steenberge, MS , Sean P. Lyden, MD , Matthew J. Eagleton, MD , Sunita D. Srivastava, MD , Timur P. Sarac, MD , Rebecca L. Kelso, MD , Daniel G. Clair, MD

Objective: With more than a decade of use of endovascular aneurysm repair (EVAR), we expect to see a rise in the number of failing endografts. We review a single-center experience with EVAR explants to identify patterns of presentation and understand operative outcomes that may alter clinical management.
