An 86-year-old woman with polycythemia vera (high allele burden of JAK2 V617F mutation: 95.3%) was referred for pulmonary edema due to severe aortic valve stenosis and was scheduled for transapical transcatheter aortic valve replacement. Because hydroxyurea administration was interrupted because of retromalleolar ulcers, preoperative blood examinations showed severe thrombocytosis (1,500 × 109/l) and neutrophilic leukocytosis (24.6 × 109/l).

JACC: CARDIOVASCULAR INTERVENTIONS. Tension Pneumopericardium Secondary to Gastropericardial Fistula Presenting as Acute Pericarditis With Cardiac Tamponade Physiology

A 44-year-old man with prior Nissen fundoplication presented with chest pain, hypotension, tachycardia, and inferior ST-segment elevation on electrocardiography. Coronary angiography demonstrated normal coronaries, and left ventriculography revealed radiolucency in the pericardial space (Figure 1, Online Video 1). Echocardiographic windows were poor. Right heart catheterization revealed diastolic equalization of pressures consistent with tamponade. He improved initially with intravenous fluids but developed recurrent hypotension and worsening sinus tachycardia. During pericardiocentesis, air and fluid were aspirated with immediate stabilization. Chest radiograph obtained before pericardiocentesis revealed pneumomediastinum (Figure 2). Computed tomography of the chest and abdomen suggested gastropericardial fistula (Figure 3), which was later repaired surgically.

A 25-year-old male presented to us 3 days after experiencing a stab knife injury to the left third intercostal space. Cardiac computed tomography (Figures 1A and 1B) showed moderate hemopericardium, autosealed perforation in the right ventricle and a large ventricular septal rupture (VSR). Two-dimensional transthoracic echocardiography with color Doppler (Figures 1C and 1D, Online Video 1) showed mild pericardial effusion, a large high muscular VSR with left to right shunt and an autosealed perforation in the right ventricle.
