CIRCULATION: CARDIOVASCULAR INTERVENTIONS. Comparison of Manual Thrombus Aspiration With Rheolytic Thrombectomy in Acute Myocardial Infarction

Guido Parodi, MD, PhD, Renato Valenti, MD, Angela Migliorini, MD, Akiko Maehara, MD, Ruben Vergara, MD, Nazario Carrabba, MD, Gary S. Mintz, MD and David Antoniucci, MD

Background—Manual thrombus aspiration (MTA) is completely ineffective in 30% of cases, and the high profiles of the catheters prevent their use in tortuous and calcified vessels. The rheolytic thrombectomy (RT) device has the potential for improved thrombus removal in acute myocardial infarction as compared with MTA. No data exist on the comparison between the 2 techniques.

CIRCULATION: CARDIOVASCULAR INTERVENTIONS. Remote Ischemic Preconditioning Improves Outcome at 6 Years After Elective Percutaneous Coronary Intervention. The CRISP Stent Trial Long-term Follow-up

William R. Davies, PhD, MRCP, Adam J. Brown, MRCP, William Watson, MRCP, Liam M. McCormick, MRCP, Nick E.J. West, MD, FRCP, David P. Dutka, MD, FRCP and Stephen P. Hoole, MA, DM, MRCP

Background—Postprocedural myocardial infarction (type 4a) has been shown to be an adverse prognostic indicator after elective percutaneous coronary intervention (PCI). The Cardiac Remote Ischemic Preconditioning in Coronary Stenting (CRISP Stent) study demonstrated that remote ischemic preconditioning reduced procedural symptoms, ECG ST-segment deviation, and cardiac troponin I release after elective PCI and reduced the major adverse cardiac and cerebral event (MACCE) rate at 6 months. We were interested to confirm if this early benefit in MACCE rate in the remote ischemic preconditioning group was sustained long-term.

CIRCULATION. Clinical and Angiographic Risk Stratification and Differential Impact on Treatment Outcomes in the Bypass Angioplasty Revascularization Investigation 2 Diabetes (BARI 2D) Trial

Maria Mori Brooks, PhD; Bernard R. Chaitman, MD; Richard W. Nesto, MD; Regina M. Hardison, MS; Frederick Feit, MD; Bernard J. Gersh, MD, PhD; Ronald J. Krone, MD; Edward Y. Sako, MD; William J. Rogers, MD; Alan J. Garber, MD; Spencer B. King III, MD; Charles J. Davidson, MD; Fumiaki Ikeno, MD; Robert L. Frye, MD for the BARI 2D Study Group

Background—The Bypass Angioplasty Revascularization Investigation 2 Diabetes (BARI 2D) trial assigned patients with type 2 diabetes mellitus to prompt coronary revascularization plus intensive medical therapy versus intensive medical therapy alone and reported no significant difference in mortality. Among patients selected for coronary artery bypass graft surgery, prompt coronary revascularization was associated with a significant reduction in death/myocardial infarction/stroke compared with intensive medical therapy. We hypothesized that clinical and angiographic risk stratification would affect the effectiveness of the treatments overall and within revascularization strata.

George Hahalis, MD, PhD, Grigorios Tsigkas, MD, Ioanna Xanthopoulou, MD, Spyridon Deftereos, MD, Antonios Ziakas, MD, PhD, Konstantinos Raisakis, MD, Christos Pappas, MD, Aggelos Sourgounis, MD, Nikos Grapsas, MD, Periklis Davlouros, MD, PhD, Anastasia Galati, MD, Theodora-Eleni Plakomyti, MD, Panagiota Mylona, MD, Ioannis Styliadis, MD, PhD, Vlassis Pyrgakis, MD, FESC and Dimitrios Alexopoulos, MD, PhD, FESC

Background—The ulnar artery is rarely selected for coronary angiography or percutaneous coronary intervention despite the expanding use of the transradial approach. We aimed to establish noninferiority of a default transulnar relative to transradial approach in terms of feasibility and safety.

JACC: CARDIVASCULAR INTERVENTIONS. Late Thrombosis After Double Versus Single Drug-Eluting Stent in the Treatment of Coronary Bifurcations. A Meta-Analysis of Randomized and Observational Studies

Marco Zimarino, MD, PhD; Alessandro Corazzini, MD; Fabrizio Ricci, MD; Marta Di Nicola, PhD; Raffaele De Caterina, MD, PhD

Objectives: This study sought to hypothesize that the higher risk of myocardial infarction (MI) documented after a routine double drug-eluting stent (DES) strategy (DDS) compared with a single DES strategy (SDS) with provisional stenting in percutaneous coronary interventions (PCI) of bifurcation lesions is driven by an increased rate of DES thrombosis.
