STROKE. Clinical Sciences. Ankle-Brachial Index and Recurrent Stroke Risk. Meta-Analysis

Ja Bin Hong, BSc*; Christopher O. Leonards, BA*; Matthias Endres, MD; Bob Siegerink, PhD; Thomas G. Liman, MD, MSc

Background and Purpose: The ankle-brachial index (ABI) is a fast, cheap, noninvasive indicator of atherosclerotic burden that may also be a predictor of stroke recurrence. In this systematic review and meta-analysis, we sought to explore ABI’s merit as a marker for stroke recurrence and vascular risk by synthesizing the data currently available in stroke literature.

SPRINGER. CT angiography versus 3D rotational angiography in patients with subarachnoid hemorrhage

R. S. Bechan, S. B. van Rooij, M. E. Sprengers, J. P. Peluso, M. Sluzewski, C. B. Majoie, W. J. van Rooij

Introduction: CT angiography (CTA) is increasingly used as primary diagnostic tool to replace digital subtraction angiography (DSA) in patients with subarachnoid hemorrhage (SAH). However, 3D rotational angiography (3DRA) has substituted DSA as a reference standard. In this prospective observational study, we compare CTA with 3DRA of all cerebral vessels in a large cohort of patients with SAH.

AMERICAN JOURNAL OF NEURORADIOLOGY. Flow-Diverter Stents for the Treatment of Saccular Middle Cerebral Artery Bifurcation Aneurysms

J. Caroff, H. Neki, C. Mihalea, F. D´Argento, H. Abdel Khalek, L. Ikka, J. Moret and L. Spelle

BACKGROUND AND PURPOSE: The flow-diverter stent has been proved a feasible, safe, and efficient technique, particularly for the treatment of large and broad-neck carotid siphon aneurysms. Wide-neck bifurcation aneurysms remain, in some cases, a challenge for neurointerventionalists. We report the outcomes of the treatment of saccular middle cerebral artery bifurcation aneurysms with flow diversion in our institution.

AMERICAN JOURNAL OF NEURORADIOLOGY. The Revascularization Scales Dilemma: Is It Right to Apply the Treatment in Cerebral Ischemia Scale in Posterior Circulation Stroke?

C. Jung, W. Yoon, S.J. Ahn, B.S. Choi, J.H. Kim and S.H. Suh

BACKGROUND AND PURPOSE: Although various revascularization scales are used in the angiographic evaluation of acute ischemic stroke, observer reliability tests of these scales have been rarely performed for posterior circulation stroke. We aimed to evaluate inter- and intraobserver variability of 2 scales, the modified Treatment in Cerebral Ischemia and the Arterial Occlusive Lesion, in posterior circulation stroke.

AMERICAN JOURNAL OF NEURORADIOLOGY. Stent Retriever Thrombectomy in Patients Who Are Ineligible for Intravenous Thrombolysis: A Multicenter Retrospective Observational Study

F. Dorn, S. Prothmann, M. Patzig, H. Lockau, C. Kabbasch, O. Nikoubashman, T. Liebig, C. Zimmer, H. Brückmann, M. Wiesmann, H. Stetefeld, H. Poppert, A. Reich, L. Kellert and G. Fesl

BACKGROUND AND PURPOSE: Intravenous thrombolysis with rtPA is the standard of care for patients with acute ischemic stroke within 4.5 hours after symptom onset. However, a considerable number of patients are ineligible for IV thrombolysis due to various contraindications. Recent studies have proved the superiority of mechanical thrombectomy for patients with large-vessel occlusions in combination with IV rtPA compared with IV rtPA alone. We aimed to demonstrate the efficacy of mechanical thrombectomy for patients who are ineligible for IV rtPA.

AMERICAN JOURNAL OF NEURORADIOLOGY. Cerebral Angiography for Evaluation of Patients with CT Angiogram-Negative Subarachnoid Hemorrhage: An 11-Year Experience

J.J. Heit, G.T. Pastena, R.G. Nogueira, A.J. Yoo, T.M. Leslie-Mazwi, J.A. Hirsch and J.D. Rabinov

BACKGROUND AND PURPOSE: CT angiography is increasingly used to evaluate patients with nontraumatic subarachnoid hemorrhage given its high sensitivity for aneurysms. We investigated the yield of digital subtraction angiography among patients with SAH or intraventricular hemorrhage and a negative CTA.

AMERICAN JOURNAL OF NEURORADIOLOGY. Favorable Bridging Therapy Based on DWI-FLAIR Mismatch in Patients with Unclear-Onset Stroke

I. Mourand, D. Milhaud, C. Arquizan, K. Lobotesis, R. Schaub, P. Machi, X. Ayrignac, O.F. Eker, A. Bonafé and V. Costalat

BACKGROUND AND PURPOSE: Standard selection criteria for revascularization therapy usually exclude patients with unclear-onset stroke. Our aim was to evaluate the efficacy and safety of revascularization therapy in patients with unclear-onset stroke in the anterior circulation and to identify the predictive factors for favorable clinical outcome.

AMERICAN JOURNAL OF NEURORADIOLOGY. Association between CYP2C19 Polymorphisms and Outcomes in Cerebral Endovascular Therapy

M. Lin, M. Todaro, J. Chan, L. Churilov, W.S. Zhu, S. Ramdave, P.J. Mitchell, R.J. Dowling, P. Kwan and B. Yan

BACKGROUND AND PURPOSE: Differing responses to clopidogrel following endovascular treatment of cerebrovascular diseases may increase the risk of vascular complications. CYP2C19 gene polymorphisms influence clopidogrel activity. We aimed to study the clinical impact of CYP2C19 gene polymorphisms in patients undergoing endovascular treatment.

STROKE. Clinical Sciences. Effects of Blood Pressure and Blood Pressure–Lowering Treatment During the First 24 Hours Among Patients in the Third International Stroke Trial of Thrombolytic Treatment for Acute Ischemic Stroke

Eivind Berge, MD; Geoffrey Cohen, MSc; Richard I. Lindley, MD; Peter Sandercock, DM; Joanna M. Wardlaw, MD; Else C. Sandset, MD; William Whiteley, MD

Background and Purpose: In patients with acute ischemic stroke, a high blood pressure or a highly variable blood pressure is a common reason for withholding thrombolytic treatment, but guidelines recommend a conservative approach to active blood pressure lowering in this setting. We have performed exploratory analyses to study the clinical effects of blood pressure and early blood pressure–lowering treatment in patients included in a randomized-controlled trial of thrombolytic treatment for acute ischemic stroke.
