JOURNAL OF VASCULAR SURGERY. Carotid-subclavian bypass and subclavian-carotid transposition in the thoracic endovascular aortic repair era

Objective: Beyond traditional indications, subclavian revascularization is increasingly performed to allow for aortic arch debranching in the setting of thoracic endovascular aortic repair (TEVAR). Endovascular treatment options for subclavian disease have emerged, perhaps altering the patient population undergoing open revascularization. We leveraged prospectively collected American College of Surgeons (ACS)-National Surgical Quality Improvement Program (NSQIP) data to delineate evolving stroke and mortality rates after carotid-subclavian bypass (CSB) and subclavian-carotid transposition (SCT) in this dynamic context.

Objectives: In patients with a ruptured abdominal aortic aneurysm (RAAA), anatomic suitability for endovascular aneurysm repair (EVAR) depends on aortic neck and iliac artery characteristics. If the aortoiliac anatomy is unsuitable for EVAR (“hostile anatomy”), open repair (OR) is the next option. We hypothesized that the death rate for OR is higher in patients with hostile anatomy than in patients with friendly anatomy.

The aim of this study was to examine the impact on survival after rEVAR when the IMPROVE protocol was initiated in a high volume abdominal aortic aneurysm (AAA) centre previously performing rEVAR.
