WORLD JOURNAL OF GASTROENTEROLOGY. Stents for colorectal obstruction: Past, present, and future

Eui Joo Kim, Yoon Jae Kim

Since the development of uncovered self-expanding metal stents (SEMS) in the 1990s, endoscopic stents have evolved dramatically. Application of new materials and new designs has expanded the indications for enteral SEMS. At present, enteral stents are considered the first-line modality for palliative care, and numerous types of enteral stents are under development for extended clinical usage, beyond a merely palliative purpose. Herein, we will discuss the current status and the future development of lower enteral stents.

WORLD JOURNAL OF GASTROENTEROLOGY. Advances in cryoablation for pancreatic cancer

Xiao-Mei Luo, Li-Zhi Niu, Ji-Bing Chen, Ke-Cheng Xu

Pancreatic carcinoma is a common cancer of the digestive system with a poor prognosis. It is characterized by insidious onset, rapid progression, a high degree of malignancy and early metastasis. At present, radical surgery is considered the only curative option for treatment, however, the majority of patients with pancreatic cancer are diagnosed too late to undergo surgery. The sensitivity of pancreatic cancer to chemotherapy or radiotherapy is also poor. As a result, there is no standard treatment for patients with advanced pancreatic cancer. Cryoablation is generally considered to be an effective palliative treatment for pancreatic cancer. It has the advantages of minimal invasion and improved targeting, and is potentially safe with less pain to the patients. It is especially suitable in patients with unresectable pancreatic cancer. However, our initial findings suggest that cryotherapy combined with 125-iodine seed implantation, immunotherapy or various other treatments for advanced pancreatic cancer can improve survival in patients with unresectable or metastatic pancreatic cancer. Although these findings require further in-depth study, the initial results are encouraging. This paper reviews the safety and efficacy of cryoablation, including combined approaches, in the treatment of pancreatic cancer.

WORLD JOURNAL OF NEPHROLOGY. Use of percutaneous nephrostomy and ureteral stenting in management of ureteral obstruction

Linda Hsu, Hanhan Li, Daniel Pucheril, Moritz Hansen, Raymond Littleton, James Peabody, Jesse Sammon

The management options for ureteral obstruction are diverse, including retrograde ureteral stent insertion or antegrade nephrostomy placement, with or without eventual antegrade stent insertion. There is currently no consensus on the ideal treatment or treatment pathway for ureteral obstruction owing, in part, to the varied etiologies of obstruction and diversity of institutional practices. Additionally, different clinicians such as internists, urologists, oncologists and radiologists are often involved in the care of patients with ureteral obstruction and may have differing opinions concerning the best management strategy. The purpose of this manuscript was to review available literature that compares percutaneous nephrostomy placement vs ureteral stenting in the management of ureteral obstruction from both benign and malignant etiologies.

WORLD JOURNAL OF NEPHROLOGY. Percutaneous nephrolithotomy in pediatric age group: Assessment of effectiveness and complications

Ender Ozden, Mehmet Necmettin Mercimek

Management of kidney stone disease in pediatric population is a challenging condition in urology practice. While the incidence of kidney stone is increasing in those group, technological innovations have conrtibuted to the development of minimally invasive treatment of urinary stone disease such as mini-percutenous nephrolitotomy (mini-PCNL), micro-PCNL, ultra mini-PCNL. In this review we tried to evaluate the effect of new teratment techniques on pediatric kidney stones.

WORLD JOURNAL OF NEPHROLOGY. Sentinel lymph node biopsy in renal malignancy: The past, present and future

Tharani Mahesan, Alberto Coscione, Ben Ayres, Nick Watkin

Sentinel lymph node biopsy (SLNB) is now an established technique in penile and pelvic cancers, resulting in a lower mortality and morbidity when compared with the traditional lymph node dissection. In renal cancer however, despite some early successes for the SLNB technique, paucity of data remains a problem, thus lymph node dissection and extended lymph node dissection remain the management of choice in clinically node positive patients, with surveillance of lymph nodes in those who are clinically node negative. SLNB is a rapidly evolving technique and the introduction of new techniques such as near infra-red fluorescence optical imaging agents and positron emission tomography/computed tomography scans, may improve sensitivity. Evidence in support of this has already been recorded in bladder and prostate cancer. Although the lack of large multi-centre studies and issues around false negativity currently prevent its widespread use, with evolving techniques improving accuracy and the support of large-scale studies, SLNB does have the potential to become an integral part of staging in renal malignancy.

JOURNAL OF HEPATOLOGY. Hepatocellular carcinoma and lifestyles

Uttara Saran, Bostjan Humar, Philippe Kolly, Jean-François Dufour

The majority of hepatocellular carcinoma occurs over pre-existing chronic liver diseases that share cirrhosis as an endpoint. In the last decade, a strong association between lifestyle and hepatocellular carcinoma has become evident. Abundance of energy-rich food and sedentary lifestyles have caused metabolic conditions such as obesity and diabetes mellitus to become global epidemics. Obesity and diabetes mellitus are both tightly linked to non-alcoholic fatty liver disease and also increase hepatocellular carcinoma risk independent of cirrhosis. Emerging data suggest that physical activity not only counteracts obesity, diabetes mellitus and non-alcoholic fatty liver disease, but also reduces cancer risk. Physical activity exerts significant anticancer effects in the absence of metabolic disorders. Here, we present a systematic review on lifestyles and hepatocellular carcinoma.

RADIOLOGY. Prospective Trial with Optical Molecular Imaging for Percutaneous Interventions in Focal Hepatic Lesions

Rahul A. Sheth, MD Ronald S. Arellano, MD Raul N. Uppot, MD Anthony E. Samir, MD, MPH Lipika Goyal, MD Andrew X. Zhu, MD, PhD Debra A. Gervais, MD Umar Mahmood, MD, PhD

Percutaneous interventions on focal hepatic lesions are among the most commonly performed procedures in interventional radiology. Biopsies of focal hepatic lesions are necessary to establish tissue diagnosis of primary or metastatic hepatic malignancy, as well as to perform genotype analysis in this era of molecularly targeted drug therapies. Thermal ablative procedures such as microwave or radiofrequency ablation likewise are important local-regional therapies for patients with small focal hepatic lesions.

AMERICAN JOURNAL OF ROENTGENOLOGY. Clinical Outcome of Transcatheter Arterial Embolization With N-Butyl-2-Cyanoacrylate for Control of Acute Gastrointestinal Tract Bleeding

Hyun Jung Koo, Ji Hoon Shin, Hwa Jung Kim, Jinoo Kim, Hyun-Ki Yoon, Gi-Young Ko and Dong Il Gwon

OBJECTIVE. The purpose of this article is to evaluate the clinical effectiveness of trans-catheter arterial embolization (TAE) with N-butyl-2-cyanoacrylate (NBCA), with or without other embolic materials for acute nonvariceal gastrointestinal tract bleeding, and to determine the factors associated with clinical outcomes.
