CARDIOVASCULAR AND INTERVENTIONAL RADIOLOGY. A Retrospective Comparison of Ultrasound-Assisted Catheter-Directed Thrombolysis and Catheter-Directed Thrombolysis Alone for Treatment of Proximal Deep Vein Thrombosis

Vladimir Y. I. G. Tichelaar, Ellen E. Brodin, Anders Vik, Trond Isaksen, Finn Egil Skjeldestad, Satish Kumar, Nora C. Trasti, Kulbir Singh, John-Bjarne Hansen

Background: Recent studies have suggested that catheter-directed thrombolysis (CDT) reduces development of post-thrombotic syndrome (PTS). Ultrasound-assisted CDT (USCDT) might enhance the efficiency of thrombolysis. We aimed to compare USCDT with CDT on efficacy, safety, development of PTS, and quality of life after long-term follow-up.

CARDIOVASCULAR AND INTERVENTIONAL RADIOLOGY. The Use of Laser Guidance Reduces Fluoroscopy Time for C-Arm Cone-Beam Computed Tomography-Guided Biopsies

Maarten W. Kroes, Marco J. L. van Strijen, Sicco J. Braak, Yvonne L. Hoogeveen, Frank de Lange, Leo J. Schultze Kool

Purpose: When using laser guidance for cone-beam computed tomography (CBCT)-guided needle interventions, planned needle paths are visualized to the operator without the need to switch between entry- and progress-view during needle placement. The current study assesses the effect of laser guidance during CBCT-guided biopsies on fluoroscopy and procedure times.

CARDIOVASCULAR AND INTERVENTIONAL RADIOLOGY. The Role of Interventional Radiology in the Treatment of Arterial Diabetic Foot Disease

Jim A. Reekers

The role of IR in treatment of diabetic foot disease and Peripheral Arterial Disease (PAD) has been growing very rapidly. It is now widely recognized that endovascular treatment can play a crucial role in prevention of amputation in diabetic patients. It is important that this role is also recognized by those who are the primary caretakers of patients with diabetes. Internationally, the International Working Group on the Diabetic Foot (IWGDF) plays an important role in establishing the evidence for endovascular treatment. The IWGDF, which was founded in 1996, is a non-profit organization with independent funding. The aim of the IWGDF is to create awareness of the disease and to improve the management and prevention of the diabetic foot. One of the main tasks of the IWGDF is to create evidence-based guidelines on the diagnosis, prognosis, and management of peripheral artery disease in patients with foot ulcers. These guidelines are updated every 4 years. In 2015, there was a new update on PAD.

JOURNAL OF VASCULAR AND INTERVENTIONAL RADIOLOGY. Superselective Conventional Transarterial Chemoembolization for Hepatocellular Carcinoma: Rationale, Technique, and Outcome

Shiro Miyayama, MD, Osamu Matsui, MD

Conventional transarterial chemoembolization with ethiodized oil and gelatin sponge (GS) particles is a standard technique for hepatocellular carcinoma. Ethiodized oil can temporarily block tumor sinusoids, portal venules, hepatic sinusoids, and arterial microcommunications. By adding GS embolization, strong ischemic effects not only on the tumor but also on the surrounding liver parenchyma can be achieved. Superselective conventional transarterial chemoembolization is mainly indicated for patients with Child-Pugh scores of 5–8, tumors ≤ 7 cm, and ≤ 5 lesions. According to a Japanese nationwide survey, the 5-year survival rate of patients with Child-Pugh class A and a single tumor was 52%.
