CATHETERIZATION & CARDIOVASCULAR INTERVENTIONS. A novel approach to reduce radial artery occlusion after transradial catheterization: Postprocedural/prehemostasis intra-arterial nitroglycerin

Surya Dharma MD, PhD, FIHA, FICA, FAPSIC, FESC, FSCAI1,*, Sasko Kedev MD, PhD, FESC, FACC2, Tejas Patel MD, FESC, FACC, FSCAI3, Ferdinand Kiemeneij MD, PhD, FESC4 andIan C. Gilchrist MD, FACC, FSCAI5

Objective: To evaluate whether administration of nitroglycerin through the sheath at the end of a transradial procedure might preserve the patency of the radial artery. Background: Despite the increasing acceptance of transradial approach, radial artery occlusion (RAO) continues to be a vexing problem of transradial access and limits utility of the radial artery as an access site in the future.

CATHETERIZATION & CARDIOVASCULAR INTERVENTIONS. Bronchoscopic guidance of endovascular stenting limits airway compression

Mohammad Ebrahim MD1, James Hagood MD2, John Moore MD1 andHowaida El-Said MD1,*

Objective: Bronchial compression as a result of pulmonary artery and aortic arch stenting may cause significant respiratory distress. We set out to limit airway narrowing by endovascular stenting, by using simultaneous flexible bronchoscopy and graduated balloon stent dilatation, or balloon angioplasty to determine maximum safe stent diameter.

Brueren MD, PhD2, Sibel Altintas Bsc5, Rutger Jan Nuis MD, PhD1, Patrick W. Serruys MD, PhD1, Leen A.F.M. van Garsse MD, PhD5, Ron T. van Domburg PhD1, Joseph Rodés Cabau MD4, Peter P.T. de Jaegere MD, PhD1 andFrits W. Prinzen PhD3,*

Objectives: The aim of the study was to investigate trends over time in the occurrence of left bundle branch block (LBBB) and permanent pacemaker implantation (PPI) after transcatheter aortic valve implantation (TAVI) with the Medtronic CoreValve System (MCS) and Edwards SAPIEN Valve (ESV). Background: TAVI-induced conduction abnormalities (TAVI-CAs) such as LBBB and the need for PPI are frequent postoperative complication. New techniques, procedural refinements, and increased awareness are focused on the reduction of these abnormalities.

AMERICAN HEART JOURNAL. Incidence of strict versus nonstrict left bundle branch block after transcatheter aortic valve replacement

Frida Sundh, Jacob Simlund, John Kevin Harrison, MD, G. Chad Hughes, MD, John Vavalle, MD, Charles Maynard, PhD, David G. Strauss, MD, PhD, Galen S. Wagner, MD, Martin Ugander, MD, PhD

Background: Up to one-third of patients diagnosed with left bundle branch block (LBBB) by conventional electrocardiographic (ECG) criteria are misdiagnosed. Strict LBBB shows decreased left ventricular pumping efficiency compared with nonstrict LBBB. However, no previous study has evaluated the frequency of strict LBBB after transcatheter aortic valve replacement (TAVR). The aim of this study was to determine the incidence of developing strict versus nonstrict LBBB after TAVR and test the hypothesis that preprocedure QRS duration does not predict strict LBBB but predicts development of nonstrict LBBB.

Background: Tricuspid regurgitation (TR) and right ventricular (RV) dysfunction adversely affect outcomes in patients with heart failure or mitral valve disease, but their impact on outcomes in patients with aortic stenosis treated with transcatheter aortic valve replacement has not been well characterized.

CIRCULATION: CARDIOVASCULAR INTERVENTIONS. Structural Heart Disease. Percutaneous Tricuspid Valve Implantation

Andreas Eicken, MD, PhD, FESC, Stephan Schubert, MD, PhD, Alfred Hager, MD, PhD, FESC, Jürgen Hörer, MD, PhD, Doff B. McElhinney, MD, John Hess, MD, PhD, FESC, Peter Ewert, MD, PhD and Felix Berger, MD, PhD

Background: Severe tricuspid valve (TV) dysfunction may lead to surgical TV replacement with a biological valve prosthesis in patients with congenital heart disease. To expand the lifetime of this valve and reduce the number of surgeries, percutaneous TV implantation (PTVI) may be an effective alternative to repeated surgery. We report on our 2-center experience with PTVI.

CIRCULATION: CARDIOVASCULAR INTERVENTIONS. Structural Heart Disease. Prosthetic Valve Endocarditis After Transcatheter Aortic Valve Implantation

Niels Thue Olsen, MD, PhD, Ole De Backer, MD, PhD, Hans G.H. Thyregod, MD, Niels Vejlstrup, MD, Henning Bundgaard, MD, DMSc, Lars Søndergaard, MD, DMSc and Nikolaj Ihlemann, MD, PhD

Background: Transcatheter aortic valve implantation (TAVI) is an advancing mode of treatment for inoperable or high-risk patients with aortic stenosis. Prosthetic valve endocarditis (PVE) after TAVI is a serious complication, but only limited data exist on its incidence, outcome, and procedural risk factors.

CIRCULATION: CARDIOVASCULAR INTERVENTIONS. Structural Heart Disease. Early Aortic Transcatheter Heart Valve Thrombosis

Tina Leetmaa, MD*, Nicolaj C. Hansson, MD*, Jonathon Leipsic, MD, Kaare Jensen, MD, Steen H. Poulsen, MD, Henning R. Andersen, MD, Jesper M. Jensen, MD, John Webb, MD, Philipp Blanke, MD, Mariann Tang, MD and Bjarne L. Nørgaard, MD

Background: Early stent valve thrombosis after transcatheter aortic valve implantation (TAVI) is a rare complication, which is diagnosed based on the appearance of clinical symptoms of heart failure and echocardiographic findings. After TAVI, transthoracic echocardiography is performed to assess transcatheter heart valve (THV) function. However, preliminary reports indicate the potential additive clinical value of multidetector computed tomography (MDCT) for the diagnosis of THV thrombosis. We sought to determine the value of MDCT for the diagnosis of THV thrombosis and the frequency of this complication after balloon-expandable TAVI.
