CIRCULATION: CARDIOVASCULAR INTERVENTIONS. Coronary Physiologic Assessment and Imaging. Transcatheter Replacement of Stenotic Aortic Valve Normalizes Cardiac–Coronary Interaction by Restoration of Systolic Coronary Flow Dynamics as Assessed by Wave Intensity Analysis

M. Cristina Rolandi, PhD, Esther M.A. Wiegerinck, MD, Lorena Casadonte, MSc, Ze-Yie Yong, MD, PhD, Karel T. Koch, MD, PhD, Marije Vis, MD, PhD, Jan J. Piek, MD, PhD, Jan Baan Jr, MD, PhD, Jos A.E. Spaan, PhD and Maria Siebes, PhD

Background: Aortic valve stenosis (AS) can cause angina despite unobstructed coronary arteries, which may be related to increased compression of the intramural microcirculation, especially at the subendocardium. We assessed coronary wave intensity and phasic flow velocity patterns to unravel changes in cardiac–coronary interaction because of transcatheter aortic valve implantation (TAVI).

CIRCULATION: CARDIOVASCULAR INTERVENTIONS. Cardiac Catheterization. Predictors of Outcome After Alcohol Septal Ablation for Hypertrophic Obstructive Cardiomyopathy. An Echocardiography and Cardiovascular Magnetic Resonance Imaging Study

Minjie Lu, MD, PhD*, Hui Du, MD*, Zhan Gao, MD, Lei Song, MD, Huaibing Cheng, PhD, Yan Zhang, MD, Gang Yin, BS, Xiuyu Chen, PhD, Jian Ling, MD, Yong Jiang, MD, Hao Wang, MD, Jinghui Li, MD, Jinghan Huang, MD, Zuoxiang He, MD, PhD and Shihua Zhao, MD, PhD, FESC

Background: Alcohol septal ablation (ASA) is used for treatment of medically refractory hypertrophic obstructive cardiomyopathy patients with severe left ventricular outflow tract (LVOT) obstruction. The current recommendations restrict the procedure to adults with favorable anatomy and no other coexisting surgically amenable disease. Some patients remain symptomatic with residual LVOT obstruction, thus better patient selection is required.

Mamas A. Mamas, BM BCh, DPhil; James Nolan, MD; Mark A. de Belder, MD; Azfar Zaman, MD; Tim Kinnaird, MD; Nick Curzen, BM, PhD; Chun Shing Kwok, MBBS, MSc, BSc; Iain Buchan, MD; Peter Ludman, MD; Evangelos Kontopantelis, PhD; on behalf of the British Cardiovascular Intervention Society (BCIS) and the National Institute for Clinical Outcomes Research (NICOR)

Background: The transradial access (TRA) site has become the default access site for percutaneous coronary intervention in the United Kingdom, with randomized trials and national registry data showing reductions in mortality associated with TRA use. This study evaluates regional changes in access site practice in England and Wales over time, examines whether changes in access site practice have been uniform nationally and across different patient subgroups, and provides national estimates for the potential number of lives saved or lost associated with regional differences in access site practice.

INTERNATIONAL JOURNAL OF CARDIOLOGY. A comparison between two different definitions of contrast-induced acute kidney injury in patients with ST-segment elevation myocardial infarction undergoing primary percutaneous coronary intervention

Marco Centola, Stefano Lucreziotti, Diego Salerno-Uriarte, Carlo Sponzilli, Giulia Ferrante, Roberta Acquaviva, Diego Castini, Marianna Spina, Federico Lombardi, Mario Cozzolino, Stefano Carugo

Background: Contrast-induced acute kidney injury (CI-AKI) is associated with significantly increased mortality after primary percutaneous coronary intervention (pPCI) for ST-segment elevation myocardial infarction (STEMI). The prognostic value of CI-AKI depends on the definitions used to define it. We compare the predictive accuracy of long-term mortality of two definitions of CI-AKI on consecutive patients undergoing pPCI for STEMI.

INTERNATIONAL JOURNAL OF CARDIOLOGY. Coronary surgery is superior to drug eluting stents in multivessel disease. Systematic review and meta-analysis of contemporary randomized controlled trials

Umberto Benedetto, Mario Gaudino1, Colin Ng, Giuseppe Biondi-Zoccai, Fabrizio D´Ascenzo, Giacomo Frati, Leonard N. Girardi, Gianni D. Angelini, David P. Taggart

Objective: Current randomized controlled trials (RCTs) comparing percutaneous coronary intervention with drug eluting stent (DES-PCI) with coronary artery bypass grafting (CABG) in multivessel disease are underpowered to detect a difference in hard clinical end-points such as mortality, myocardial infarction and stroke. We aimed to overcome this limitation by conducting a meta-analysis of contemporary RCTs.
